The Value of Contrast-enhanced CT Combined with DWI in Grading Diag-nosis of Pancreatic Neuroendocrine Tumors
Objective To explore the grading diagnostic value of contrast-enhanced CT combined with magnetic reso-nance diffusion-weighted imaging(DWI)for pancreatic neuroendocrine tumors(pNEN).Methods 68 pNEN patients ad-mitted to Hai'an People's Hospital from June 2020 to June 2024 were retrospectively selected as research subjects.All patients underwent enhanced CT and DWI detection,and the differences in CT parameters and ADC values of pNEN patients with dif-ferent pathological grades were compared.The diagnostic value of enhanced CT combined with DWI was analyzed using the re-ceiver operating curve(ROC).Results Comparison of CT parameters:There were statistically significant differences in tumor shape,edge,main pancreatic duct dilation,bile duct dilation,and CT absolute enhancement values between portal ve-nous phase and arterial phase(P<0.05).There were significant differences in ADC values among patients with pNEN of dif-ferent pathological grades,and patients with grade G1 NET were significantly higher than patients with other grades of pNEN,and patients with grade G2 NET were significantly higher than patients with NEC(P<0.05).ROC curve analysis showed that the combined prediction AUC of enhanced CT and DWI for G1 and G2 grade NETs was 0.932(95%CI:0.871~0.993,P<0.01),the sensitivity was 75.84%,and the specificity was 95.20%;G2 and G3 grade NETs and The combined predic-tion AUC of NEC enhanced CT and DWI was 0.943(95%CI:0.877-1.000,P<0.01),the sensitivity was 92.92%,and the specificity was81.84%.Conclusion Contrast-enhanced CT combined with DWI can be used for pathological grading di-agnosis of pNEN.
