低温等离子下鼻甲射频消融术对慢性肥厚性鼻炎患者鼻腔结构、鼻黏液纤毛清除功能及气道阻力的影响

目的 分析低温等离子下鼻甲射频消融术对慢性肥厚性鼻炎患者鼻腔结构、鼻黏液纤毛清除功能及气道阻力的影响.方法 选择2023 年8 月至2024 年8 月收治的 100 例慢性肥厚性鼻炎患者,以案例排序的奇偶性分为观察组和对照组各50 例,观察组行低温等离子下鼻甲射频消融术治疗,对照组行鼻内镜下下鼻甲部分切除术治疗.比较2 组治疗效果、手术指标,以及治疗前后鼻腔结构、鼻黏液纤毛清除功能及气道阻力相关指标.结果 观察组治疗总有效率高于对照组(P<0.05).观察组手术时间、术后鼻腔疼痛时间、住院时间均短于对照组,术中出血量少于对照组(P<0.01).治疗后,观察组宽敞侧、狭窄侧下鼻甲最下缘与鼻底的垂直距离、下鼻甲最内缘与鼻中隔的水平距离均长于对照组,下鼻甲水平方向上的最大厚度均低于对照组,差异有统计学意义(P<0.05,P<0.01).治疗后,观察组黏液纤毛清除率、黏液纤毛清除速率均高于对照组,糖精清除时间短于对照组(P<0.05,P<0.01).治疗后,2组呼吸总阻抗Z5、总气道阻力R5、共振频率均降低,且观察组低于对照组(P<0.05,P<0.01).结论 在慢性肥厚性鼻炎治疗中实施低温等离子下鼻甲射频消融术可取得显著效果,可优化手术指标,促进患者鼻腔、气道、鼻黏液组织结构功能恢复.
Effects of Low Temperature Plasma Radiofrequency Ablation of Turbinate on Nasal Structure,Nasal Mucociliary Clearance and Airway Resistance in Patients with Chronic Hyperplastic Rhinitis
Objective To analyze the effects of low temperature plasma radiofrequency ablation of turbinate on nasal structure,nasal mucociliary clearance and airway resistance in patients with chronic hyperplastic rhinitis.Methods 100 pa-tients with chronic hyperplastic rhinitis treated from August 2023 to August 2024 were selected and divided into observation group and control group,50 cases in each group according to the parity of cases.The observation group was treated with low temperature plasma radiofrequency inferior turbinate ablation,and the control group was treated with nasal endoscopic partial resection of inferior turbinate.The therapeutic effect,surgical indexes,nasal structure,mucociliary clearance function and airway resistance were compared between the two groups before and after treatment.Results The total effective rate of obser-vation group was higher than that of control group(P<0.05).The operation time,postoperative nasal pain time and hospital stay in the observation group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group(P<0.01).After treatment,the vertical distance between the bottom margin of the inferior turbinate and the nasal base and the horizontal distance between the innermost margin of the inferior turbinate and the nasal septum in the observation group were longer than those in the control group,and the maximum horizontal thickness of the inferior turbinate was lower than that in the control group,with statistical significance(P<0.05,P<0.01).After treatment,the mucociliary clearance rate and mucociliary clearance rate of observation group were higher than those of control group,and the saccharin clearance time was lower than that of control group(P<0.05,P<0.01).After treatment,the total respiratory impedance Z5,total airway resistance R5 and resonance frequency of 2 groups were decreased,and the observation group was lower than the control group(P<0.05,P<0.01).Conclusion In the treatment of chronic hypertrophic rhinitis,low temperature plasma radiofrequency ablation of turbinate can achieve remarkable results,optimize the surgical indicators,and promote the structural and functional recovery of nasal cavity,airway and nasal mucus.
