Predictive Value of Serum Insulin-like Growth Factor Binding Protein-3 Combined Placental Microvascular Index for Fetal Growth Restriction in Elderly Patients with Preeclampsia
Objective To investigate the predictive value of serum insulin-like growth factor binding protein-3(IGFBP-3)combined with placental microvascular index on fetal growth restriction(FGR)in patients with high-grade pre-eclampsia(PE).Methods A total of 152 elderly PE patients(PE group)and 100 healthy pregnant women(control group)admitted from January 2021 to May 2023 were selected,and the elderly PE patients were divided into FGR group(n=34)and non-FGR group(n=118)according to the occurrence of FGR.Serum IGFBP-3 levels and placental microvascular index[vascularization index(VI),blood flow index(FI),vascularization blood flow index(VFI)]were measured.Multivariate Logistic regression and receiver operating characteristic(ROC)curve were used to analyze the influencing factors of FGR in elderly PE patients and the predictive value of serum IGFBP-3 combined with placental microvascular index for FGR in elderly PE patients.Results Compared with the control group,the serum IGFBP-3 level in PE group was increased,and the VFI,FI and VI levels were decreased(P<0.05).The incidence of FGR in elderly PE patients was22.37%(34/152).The in-dependent risk factors for FGR in elderly PE patients were the increase of 24 h urinary protein and IGFBP-3,and the inde-pendent protective factors were the increase of VFI,FI and VI(P<0.05).The area under the curve of FGR predicted by se-rum IGFBP-3 combined with placental microvascular index was 0.904 in elderly PE patients,which was larger than 0.799,0.753,0.714 and 0.734 predicted by serum IGFBP-3,VFI,FI and VI alone(P<0.05).Conclusion Serum IGFBP-3 levels increased in elderly PE patients,while VFI,FI and VI levels decreased,which was independently correlated with FGR in elderly PE patients.Serum IGFBP-3 combined with placental microvascular index had high predictive value.
