Comparison of the Efficacy and Safety of Endoscopic Surgery for Thyroid Papillary Carcinoma Through Submental and Oral Vestibular Approaches Based on Propensity Score Matching Study
Objective To compare the effectiveness and safety of endoscopic surgery for thyroid papillary carcinoma through submental and oral vestibular approaches based on propensity score matching.Methods Clinical data of 150 patients with thyroid micropapillary carcinoma undergoing elective surgery from January 2022 to December 2023 were included.Ac-cording to the operation plan,85 cases were divided into oral group and 65 cases were submental group.Propensity score matching method was used to match the confounding factors before balanced treatment at a ratio of 1∶1,and 48 cases in each group were successfully matched.Perioperative indexes,pain degree,aesthetics,complications and recurrence rate were com-pared between the two groups.Results The length of operation in submental group was longer than that in oral group(P<0.05).There were no significant differences in intraoperative blood loss,number of central lymph nodes dissection,24 h postoperative drainage volume and length of hospital stay between the two groups(P>0.05).24 h after surgery,the submen-tal pain visual analogue scale(VAS)score was lower than that of oral group(P<0.05).72 h after operation,there was no significant difference in pain VAS score between the two groups(P>0.05).At 1 and 3 months after surgery,the Vancouver Scar Rating Scale score of oral group was lower than that of submental group(P<0.05).There was no significant difference in complication rate and recurrence rate between 2 groups(P>0.05).Conclusion Endoscopic thyroidectomy through oral vestibular approach and submental approach have similar results in the treatment of minimal papillary thyroid carcinoma.En-doscopic thyroidectomy through oral vestibular approach has better aesthetic appearance,while endoscopic thyroidectomy through submental approach has lower pain and shorter operation time.
