Analysis on semen quality and its influencing factors in 9 852 male infertility patients
Objective To analyze the semen quality of 9 852 male infertility patients and to discuss its influencing factors.Methods The semen quality was examined in 9 852 male infertility patients who visited Wenzhou Hospital of Traditional Chinese Medicine from January 2018 to December 2022,and 673 healthy fertility males were selected as control group.The semen routine indexes inducle semen volume,viscosity,agglutination,liquefied time,pH value,sperm concentration,total number of sperm,total sperm motility rate[progressively motile sperm(PR)and non-progressively motile sperm(NP)]were analyzed and compared between two groups according to testing year,age and abstinence days.Multiple linear regression analysis was conducted to analyze the effects of genitourinary infection,living habits,occupational characteristics on the semen parameters.Results The abnormal rate of those semen paramenters in infertility patients from high to low was:the percentage of normal sperm,PR,viscosity,pH value,iquefied time,total sperm motility rate,sperm concentration,total number of sperm,semen volume and agglutination.In infertile group the values of sperm concentration,total number of sperm,total sperm motility rate,PR and the percentage of normal sperm which were all lower than those in the control group(P<0.01);while pH value,liquefied time were higher than control group(P<0.01).From 2018 to 2022,the sperm concentration,total number of sperm,and the percentage of normal sperm showed a decreasing trend year by year(all P<0.05).The total number of sperm,total sperm motility rate and PR decreased with the increase of age(all P<0.001).There were significant differences in sperm concentration,total sperm motility rate,total number of sperm and PR among patients with different abstinence days(all P<0.05).Multiple linear regression analysis showed that genitourinary infection,smoking,drinking,occupational exposure to heat and irritating environment were associated with sperm concentration,total number of sperm,total sperm motility rate,PR and the percentage of normal sperm.Sedentary behavior was associated with total sperm motility rate,PR,and percentage of normal sperm.Conclusion The sperm concentration,total number of sperm and the percentage of normal sperm in male infertility patients show a decreasing trend from 2018 to 2022.Age,abstinence days,genitourinary infection(prostatitis,ureaplasma urealyticum infection),unhealthy lifestyle(smoking,drinking,sedentary),and occupational exposure(heat,irritating environment)are influencing factors for semen quality in infertile men.
