Clinical characteristics and prognosis of hypertensive patients with cirrhotic esophagogastric variceal bleeding
Objective To evaluate the clinical manifestation and prognosis of hypertensive patients with cirrhotic esophagogastric variceal bleeding(EVB).Methods The clinical data of 115 cirrhotic patients receiving endoscopic therapy for EVB of Sir Run Run Shaw Hospital from October 1996 to April 2021 was retrospectively analyzed,including 17 cases with arterial hypertention(AH group)and 98 cases with normal blood pressure(NBP group).The clinical manifestations,laboratory testing results and prognosis of patients were compared between two groups.Results The age of patients in AH group[(65.4±2.2)years]was significantly higher than that in NBP group[(54.7±1.1)years](P<0.01).The proportion of patients with autoimmune liver disease in AH group(17.6%)was significantly higher than that in NBP group(3.1%),while the serum albumin level in AH group[(32.4±1.5)g/L]was significantly higher than that in NBP group[(30.1±0.6)g/L](both P<0.05).The incidence of re-bleeding after endoscopic hemostasis in AH group(5.9%)was significantly lower than that in NBP group(7.1%)(P<0.01).Conclusion Hypertensive patients combined with cirrhotic esophagogastric variceal bleeding tend to be older and have higher serum albumin level,lower incidence of re-bleeding and higher proportion of autoimmune liver disease.The results suggest that hypertensive patients may have a slower progression of the chronic liver disease with better liver function and a better prognosis.
