Sensitivity of C-TIRADS,elasticity scoring system and Bethesda system in diagnosis of malignant thyroid nodules
Objective To analyze the diagnostic sensitivity of Chinese-thyroid imaging reporting and data system(C-TIRADS),elasticity scoring system and the Bethesda system for reporting thyroid cytopathology(TBSRTC)in diagnosis of malignant thyroid nodules.Methods A total of 122 patients with 134 thyroid nodules admitted to the First Hospital of Jiaxing from June 2020 to June 2022 were enrolled.All patients underwent routine ultrasonography with elasticity scoring and C-TIRADS classification,and TBSRTC grading was performed after thyroid fine needle aspiration biopsy(FNAB).Taking pathological diagnosis as the gold standard,the diagnostic value of different diagnostic methods for thyroid nodule properties was analyzed.Results The sensitivity,accuracy and negative predictive value of TBSRTC classification were 0.903,0.881 and 0.714,which were higher than those of elasticity scores(0.709,0.754,0.483)and C-TIRADS(0.718,0.716,0.431)(all P<0.05).The sensitivity of elasticity score and C-TIRADS combined with TBSRTC in the diagnosis of malignant nodules was 0.971,which was higher than that of elasticity score combined with C-TIRADS(0.757),elasticity score combined with TBSRTC(0.903)and C-TIRADS combined with TBSRTC(0.903)(all P<0.05).The sensitivity of elasticity score combined with TBSRTC in diagnosis of malignant nodules>C-TIRADS class 4B was 0.973,which was higher than that of elasticity score(0.811)or TBSRTC(0.878)alone(P<0.05).Conclusion TBSRTC alone is better than elasticity score and C-TIRADS in the diagnosis of malignant thyroid nodules,and the combination of the three methods can improve the diagnosis effect.Elasticity score combined with TBSRTC has good diagnostic value in evaluating the nature of nodules>C-TIRADS class 4B.
