Serum levles of AFP,AFP-L3,GP73 and DCP in chronic hepatitis B and liver cancer
Objective To investigate the serum levels of alpha-fetoprotein(AFP),alpha-fetoprotein L3(AFP-L3),golgi protein 73(GP73)and des-γ-carboxy prothrombin(DCP)in chronic hepatitis B and liver cancer. Methods A total of 78 patients with liver cancer(LC group)and 90 patients with chronic hepatitis B(CHB group)admitted in Affiliated Jinhua Hospital of Zhejiang University School of Medicine from January 2022 to March 2023 were enrolled, and 70 healthy individuals who underwent physical examinations in our hospital during the same period were chosen as the normal control group.Serum levels of AFP,AFP-L3,GP73,and DCP were measured in all subjects. The serum levels and positive detection rates of above indicators were compared among three groups. The diagnostic efficacy of the above indicators for liver cancer was analyzed, and their levels before and after surgery were compared. Results There were significant differences in serum AFP,AFP-L3,GP73,DCP levels and positive rates among three groups:GP73、DCP levels highest in LC group and lowest in control group(all P<0.05).The areas under the ROC curve(AUC)of AFP,AFP-L3,GP73,and DCP for diagnosing liver cancer were 0.745,0.916,0.753 and 0.785,respectively.With the optimal cut-off value of 9.35%,AFP-L3 had the highest sensitivity(0.859);with the optimal cutoff value of 9.37 ng/mL,AFP had the highest specificity(0.900)with a relatively low sensitivity(0.564).The levels of serum AFP,AFP-L3,GP73,and DCP in live cancer patients decreased after surgery compared with those before surgery(all P<0.05).Conclusion The serum AFP,AFP-L3,GP73 and DCP levels are increased in chronic hepatitis B and liver cancer;the detection of above indicators may have potential value for diagnosis and prognosis of liver cancer patients.
