目的 观察显微根尖手术对根尖周炎的治疗效果.方法 回顾性选取2019年1月至2023年1月在绍兴市口腔医院诊治的根尖周炎患者75例.其中38例患者行显微根尖手术治疗,为观察组;另37例患者行传统根尖手术治疗,为对照组.比较两组患者术后7 d疼痛发生情况、术后并发症发生情况、术前和术后7 d牙周指标,包括菌斑指数(PLI)、牙龈出血指数(GBI)和牙周袋探诊深度(PD);比较两组患者术后12个月疗效.结果 观察组患者术后7 d疼痛发生率低于对照组(7.89％比27.03％,P＜0.05),术后并发症发生率小于对照组(7.89％比37.84％,P＜0.05).两组患者术后7 d PLI、GBI和PD均低于术前(均P＜0.05),且观察组患者术后7 d PLI、GBI和PD均低于对照组(均P＜0.05).观察组患者术后12个月治疗总有效率高于对照组(97.37％比78.38％,P＜0.05).结论 显微根尖手术治疗根尖周炎效果显著,且可显著改善牙周指标,降低术后疼痛和并发症发生风险,值得临床推广应用.
Efficacy of micro-apical surgery for patients with periapical periodontitis
Objective To investigate the efficacy of micro-apical surgery for patients with periapical periodontitis.Methods A total of 75 patients with periapical periodontitis treated in Shaoxing Stomatological Hospital from January 2019 to January 2023 were enrolled,including 38 patients receiving microscopic apical surgery(study group)and 37 patients received traditional apical surgery(control group).Compare,and preoperative and postoperative The periodontal indicators before and 7d after operation,including plaque index(PLI),gingival bleeding index(GBI),and periodontal pocket probing depth(PD)were compared;the postoperative incidence of pain and complications,as well as the efficacy at 12 months after surgery were compared between two groups.Results The incidence of postoperative pain and complications in study group were significantly lower than those in control group(7.89％vs.27.03％and 7.89％vs.37.84％,P＜0.05).The PLI,GBI,and PD at 7d after surgery were significantly lower than those before surgery in both groups(all P＜0.05);while the postoperative PLI,GBI and PD in study group were lower than those in the control group(all P＜0.05).At 12 months after surgery,the effective rate of treatment in the study group was significantly higher than that in the control group(97.37％vs.78.38％,P＜0.05).Conclusion Micro-apical surgery has a better efficacy than traditional apical surgery in treatment of patients with periapical periodontitis,which is worthy of clinical promotion and application.
