Analysis on influencing factors of prolonged average length of stay in patients undergoing total hip arthroplasty patients based on DRGs indicators
Objective To analyze the influencing factors of prolonged average length of stay (ALOS) in patients undergoing total hip arthroplasty (THA) based on diagnosis related groups (DRGs) indicators. Methods A retrospective analysis was conducted on 399 discharged patients who received THA at Jinhua Municipal Central Hospital from January to December 2022. Patients were divided into two groups based on their hospital stay:≤14 days group (260 cases) and>14 days group (139 cases). The clinical characteristics,DRGs data,and perioperative complications of the two groups were compared. Results The proportions of patients with diabetes,cardiovascular diseases,and cerebrovascular diseases in the>14 days group were higher than those of the ≤14 days group;the proportions of avascular necrosis of the femoral head was lower than that of the ≤14 days group;the costs of blood transfusions,nursing care,rehabilitation,and out-of-pocket expenses were also higher than those of the ≤14 days group;the operating room stay was longer than that of the ≤14 days group;and the incidences of perioperative constipation,hypoproteinemia,pulmonary embolism,lower limb venous thrombosis,and urinary tract infections were higher than those of the ≤14 days group,the differences were all statistically signifcant (all P<0.05). Conclusion The presence of comorbidities,prolonged surgery duration,and the occurrence of perioperative complications are the primary factors influencing the length of hospital stay for THA patients. These factors also significantly increase medical costs and labor expenditures. Therefore,it is essential to refer to DRGs monitoring data to make effective strategies to optimize diagnosis and treatment processes and improve hospital service capacity and efficiency.
