Factors related to abnormal bone metabolism markers in patients with gouty arthritis
Objective To investigate the factors related to serum bone metabolism markers in patients with gouty arthritis (GA).Methods Sixty GA patients who underwent dual-energy CT in the First Affiliated Hospital of Henan University of Traditional Chinese Medicine from January to December 2023 were enrolled,including 30 cases with gouty tophi (tophi group) and 30 cases without gouty tophi (non-tophi group);15 healthy individuals were selected as controls.The serum levels of uric acid (UA),ALT,AST,serum creatinine (Scr),FPG,LDL,HDL,TG and TC were measured in all subjects;the bone metabolism markers receptor of activator of NF-κB ligand (RANKL),osteoprotegerin (OPG) and human type Ⅰ collagen cross-linked C-terminal peptide (CTX) were detected with ELISA and the correlation analysis on the above indicators were conducted.Results The results indicate that the serum UA,Scr,LDL,TG and TC levels in tophi group were significantly higher,and HDL was lower than those in control group (all P<0.05);while serum levels of UA,TG and TC in non-tophi group were significantly higher than those in control group (all P<0.05).Patients in tophi group had higher Scr,TG,and TC levels and lower HDL levels than those in non-tophi group (all P<0.05).The RANKL,OPG and CTX levels in tophi group were higher than those in control group (all P<0.05),and the OPG/RANKL was lower than that of control group (P<0.05).The RANKL level in non-tophi group was higher than that in control group (P<0.05),while RANKL and OPG levels in tophi group were significantly higher than those in non-tophi group (all P<0.05).Correlation analysis showed that RANKL was positively correlated with UA,Scr,LDL,TG and TC (all P<0.05);OPG was positively correlated with UA,Scr,LDL,TG,TC,and negatively correlated with AST (all P<0.05);OPG/RANKL was negatively correlated with Scr (P<0.05).Conclusion GA patients exhibit high levels of UA,along with abnormalities in bone metabolism markers,lipid metabolism disorders,and decreased renal function,particularly pronounced in those with tophi.Bone metabolic markers RANKL,OPG and OPG/RANKL are correlated with the serum levels of UA,Scr,LDL,HDL,TG and TC.
