严季澜教授认为,胃脘痛病机关键在于胃失通降,发病初期多属肝郁气滞,以四逆散加减治疗,后期多属寒热错杂,虚实兼挟,以辛开苦降之丰夏泻心汤加减治疗.临证中严师非常重视患者嗳腐、吞酸及大便情况,并参考现代医学检查结果,衷中参西,以指导临证辨治.
Abstract
In pro. Yan Jilan' s view, key mechanism of epigastric pain lies in stomach losing stretching-fall; in primary stage of attack, it often belongs to liver depression and Qi block, treated with Sim Powder; in later stage, it often belongs to mixed coldness and heat, and mixed deficiency and excess,treated with Pinellia Xiexin Decoction pungent and bitter for opening and descending. In clinic, master Yan pays attention to patients' belching, acid regurgiution and defecation, referring to modern medical tests, combining with western medicine, to guide clinical differentiating treatment.
