液基薄层细胞制片技术对检测脑脊液脱落细胞学的应用

目的:探讨液基薄层细胞制片技术在脑脊液脱落细胞学检查中的应用。方法用传统涂片和TCT制片同时对172例脑脊液脱落细胞学检查送检标本进行制片,比较两种方法对癌细胞阳性的检出率,并观察其病理结果。结果 TCT制片对脑脊液脱落细胞学癌细胞阳性检出率高于传统涂片,差异有统计学意义(P<0.05)。传统涂片细胞量极少,核仁不明显,着色较深,存在细胞变形现象;T C T制片细胞形态饱满,细胞核呈圆形、椭圆形,核膜清晰且光滑,染色质均细,着色浅,可见细小的核仁。细胞质在两种涂片未见明显区别。结论 TCT制片技术对脑脊液脱落细胞学癌细胞阳性检出率及诊断准确率高,漏诊率低。
The application of thin-layer cytological test in the examination of cerebrospinal fluid
Purpose: To discuss the application of Thin-layer cytological test (TCT) in the examination of cerebrospinal fluid. Method: 172 cases of exfoliative cytological specimens of cerebrospinal fluid were examined by traditional smear method and TCT method to compare the positive detection rates of cancer celland observe the pathological results. Results: the positive detection rates by TCT were significantly higher than those by traditional smear for both cerebrospinal fluid specimens (P < 0.05). In the traditional smear, very few cells were observed with less clear karyosome, strong staining and cytomorphosis; in the TCT smear, cells remained intact with circular or el iptic cellnucleus;defined nuclear membrane evenly distributed chromatin and easily observed leptos karyosomes. There was no significant difference in cytoplasm between these two methods. Conclusion: TCT smear can increase the accuracy of detection rate of exfoliative cells in Cerebrospinal Fluid samples and reduce the chance of false negative diagnosis.
