摘要
目的：分析门诊药房调剂差错出现的原因，找出相应的防范对策。方法：对我院2009年-2013年出现的72例门诊处方调剂出现差错进行分析，分析其出现差错的主要原因，建立有效的管理机制。结果：本次研究显示，出现差错的主要原因包括：药师出现差错36例，占50%；医师原因18例，占25%；患者原因16例，占22.22%，其它原因2例，占2.78%。结论：药房必须不断完善规章制度，规范调剂操作的流程，改善药房工作环境，提高药师和医师的业务水平，形成工作责任心，降低调剂错误发生率。
Abstract
Objective: To analyze the reasons for outpatient pharmacy dispensing error occurs, find the appropriate countermeasures. Methods: The emergence of our hospital in 2009 and 2013 appear in 72 cases outpatient prescription dispensing error analysis and the main reason for their errors, and to establish an effective management mechanism. Results: This study shows that the main reason for errors include: Pharmacists errors in 36 cases, accounting for 50%; physicians reason in 18 cases, accounting for 25%; reasons 16 patients, accounting for 22.22%, other causes in 2 cases, accounting for 2.78% . Conclusion: The pharmacy must continue to improve rules and regulations, standardize processes swap operations, improve pharmacy work environment, improve the professional level of pharmacists and physicians, formed sense of responsibility, reducing the incidence of dispensing errors.
