Objective To explore effectiveness of the community interventions for patients with hypertension by use of community health service centers. Methods Selected eighty patients with hypertension from Tianchang South Community Health Service Center between October 2012 to October 2013, a systematical community intervention containing health education intervention, psychological intervention, lifestyle intervention, drug intervention and health education of family members of patients with hypertension were carried out in patients with hypertension, then the awareness rate of hypertension-related knowledge , changes in blood pressure and related metabolic parameters and changes in bad habits were observed and compared before and after the intervention to evaluate the effectiveness of hypertension on community intervention. Results The awareness rate of hypertension-related knowledge after the intervention was significantly higher than before the intervention, the differences were statistical y significant (P <0.05).The blood pressure and metabolic parameters after the intervention patients were lower than before the intervention, and various of metabolic parameters were improved significantly, the differences were statistical y significant (P <0.05). Patients with bad habits after intervention were lower than before the intervention, the differences were statistical y significant (P <0.05). Conclusion By taking hypertension community interventions can strengthen the management role of community health centers to improve understanding of the disease, the effective control of blood pressure and metabolic parameters, maintaining healthy habits , improving quality of life in patients with hypertension. So it is worthy of clinical application.
