Objective To estimate the effects and complications of Microsurgery and gamma knife radiosurgery for huge acoustic neuromas.Method 92 patients with huge acoustic neurilemoma were treated by microneurosurgery,during which facial nerve monitoring was performed. Residuary tumor was treated by gamma knife in 2-3 months after operation. The tumor margin dose was 12-14Gy with 50% isodose curve.Results Al the patients were fol ow up for 6 months to 10 years,mean 5.2 years.The tumor control rate was 96.7%.Facial nerve function retention rate was 89.1%.Hearing preservation rate was 63.6%.Conclusion Microsurgery combine gamma knife achieved good tumor control rate for treatment of huge acoustic neuromas,and reduced acoustic-facial nerve injury.
