摘要
目的:探讨经椎弓根椎体内植骨加钉棒系统内固定治疗胸腰椎骨折的临床效果。方法自2010年8月至2012年12月,采用特制植骨漏斗,经伤椎椎弓根椎体内植骨,结合短节段钉棒系统内固定,或同时行椎管减压,治疗单节段胸腰椎骨折患者89例。测量伤椎高度及后凸角度,比较手术前后Cobb角的变化,随访伤椎高度丢失情况。结果全部病例经术后1年以上随访,未发生因植骨操作引起的血管、神经损伤等相关并发症,无断钉断棒现象发生,椎体高度恢复至原椎体高度的85%以上,CT显示植骨融合良好,Cobb角丢失约1-2°。结论经椎弓根椎体内植骨加钉棒系统内固定,不仅重建了胸腰椎稳定性,恢复了伤椎高度与脊柱序列,而且明显降低了内固定失败的发生率,临床效果满意。
Abstract
objective To investigate clinical effects the vertebrae pedicle bone graft and internal fixation nail rod system for treatment of thoracolumbar fractures. Methods From August 2010 to December 2012, adopt special funnel, bone graft by vertebral pedicle vertebral body bone graft injury, combined with short segmental fixation nail stick system, or a line of vertebral canal decompression at the same time, the treatment of 89 patients with single segmental thoracolumbar fractures. Preoperative, postoperative and fol ow-up of the lateral thoracic lumbar segment is slice, measurement of injured vertebral height, comparison of injured vertebral height restoration and bone graft fusion. Results Al cases were fol ow-up after an average of more than 1 year. There were no bleed, nerve damage and others related complications caused by bone graft operation, and no broken nail rod broken phenomenon occurs, vertebral height restoration and good bone graft fusion. Conclusion Most of clinical effects is satisfactory, who treated with the vertebrae pedicle bone graft and internal fixation nail stick system. Such as rebuild the stability of thoracolumbar, recovery of injured vertebral height, spinal sequence, and significantly reduced the incidence of internal fixation failure,.
