Objective To analyze the patients out of hospital pressure ulcer related factors, to explore the community and family intervention for pressure sore prevention and nursing. Methods of statistics of the first quarter of 2012 and the second quarter to go into hospital pressure ulcer of patients with 60 cases, according to the nursing department according to Braden score table to develop pressure sores on statements of the packet to intervene in the Department, offering training and the use of telephone manner nursing instruction. Results the involvement of community intervention in patients after an occurrence of a pressure ulcer .Conclusion the implementation of community and home care can effectively reduce the incidence of pressure sores.
