Study on the Clinical Characteristics and Risk Factors of Shen Yang Deficiency Syndrome Female Based on 2 337 Cases of Community Natural Population Survey
Objective To explore the clinical characteristics,tongue and pulse characteristics and risk factors of Shen yang deficiency syndrome female,and to provide data support for objective syndrome differentiation and prevention.Methods The general information,Chinese medicine(CM)four diagnostic information,Shen yang deficiency scale information and serum samples of four community residents in Haidian District of Beijing were collected from September 2020 to November 2022.Shen yang deficiency syndrome was diagnosed based on the syndrome diagnostic criteria and scale scores,the residents who met the criteria were screened.The differences of item scores,tongue and pulse frequency distribution,basic information,living habits and detection indexes of Shen yang deficiency scale in different genders were compared.The statistically significant items were included in Logistic regression to explore the risk factors of female Shen yang deficiency syndrome,and Spearman was used to analyze the correlation between female menstrual history and Shen yang deficiency scale scores.Results A total of 2 416 residents completed the survey.Totally 2 337 residents met the standard of allotments,including 924 males and 1 413 females.Compared with male patients,the total scores and items of the Shen yang deficiency scale in female patients were higher in the scores of waist pain,knee weakness,waist and knee fear of cold,and intolerance to cold,and the scores of frequent nocturia were lower(P<0.05,P<0.01).The high-frequency tongue and pulse of female patients were white coating,greasy coating,red tongue,deep pulse,string pulse and thready pulse.The proportion of female solid tongue pulse was lower than that of male,and the proportion of virtual pulse was higher than that of male.The age and BMI of female patients were lower than those of male patients,and the proportion of smoking and drinking history was lower than that of male patients(P<0.05,P<0.01).Female patients had light taste,vitamin B12 deficiency and hypothyroidism,and TC,LDL and HDL were higher(P<0.05,P<0.01).Shen yang deficiency scores in female was negatively correlated with menopausal age(r=-0.76),and positively correlated with menarche age(r=0.05),number of pregnancies(r=0.10),and number of births(r=0.09,P<0.05,P<0.01).Age,folic acid,vitamin B12 deficiency history,hypothyroidism history,depression scale score,menopausal age,and number of pregnancies were related to the occurrence of female Shen yang deficiency syndrome(P<0.05).Age,history of folic acid and vitamin B12 deficiency,history of hypothyroidism,high score of depression scale,young age of menopause,and more pregnancies were independent risk factors for the occurrence of Shen yang deficiency syndrome in women(P<0.05).Conclusions The clinical manifestations of Shen yang deficiency syndrome female are waist pain,knee weakness,waist and knee cold fear,cold intolerance performance.A variety of tongue and pulse coexist,and mixed with deficiency and excess.There were significant differences in gender,BMI,tobacco and alcohol history,dietary taste,folate and vitamin B12 deficiency,hypothyroidism prevalence,FPG,TC,LDL and HDL.Old age,low metabolic status and abnormal reproductive history were the main risk factors.(Chinese Clinical Trial Registry,No.ChiCTR1 900025713)
