Interventional effect of early progressive exercise on bedside ultrasound monitoring of critically ill patients in the intensive care unit
Objective To explore the intervention effect of early progressive exercise on bedside ultrasound monitoring of critically ill patients in the intensive care unit(ICU).Methods A retrospective analysis was conducted on 120 mechanically ventilated patients admitted to the Emergency ICU and the Department of Critical Care Medicine of Guigang City People's Hospital from January 2020 to January 2022.Both groups received basic treatment and routine nursing.The patients were assigned into a control group(60 cases receiving routine rehabilitation training)and an observation group(60 cases receiving routine rehabilitation training and early progressive exercise)via a random number table.The incidence of ventilator-associated pneumonia(VAP),respiratory muscle function,muscle parameters,muscle strength,and grip strength were compared between the two groups during hospitalization.Results The duration of mechanical ventilation,ICU length of stay,and total length of hospital stay of the observation group were shorter than those of the control group,and the incidence of VAP was lower than that of the control group(all P<0.05).At 48 h after ICU admission,the cough peak flow and maximum inspiratory pressure of the observation group were higher than those of the control group(both P<0.05).At 1 week after ICU admission and at ICU discharge,the cross-sectional area of rectus femoris,thickness of rectus femoris,thickness of vastus intermedius,thickness of the tibialis anterior,thickness of biceps brachii,and thickness of flexor carpi radialis in the observation group were all greater than those in the control group,and the Medical Research Council score and the grip strength were better than those of the control group(all P<0.05).Conclusion Early progressive exercise is effective in bedside ultrasound monitoring of critically ill patients in the ICU,which can effectively reduce the ICU length of stay and the incidence of VAP,improve the respiratory muscle function,and enhance the muscle thickness,muscle strength and grip strength.
