Effect of a nursing model based on midwife-led counseling on the fear of childbirth and self-efficacy of primiparas
Objective To investigate the effect of a nursing model based on midwife-led counseling on the fear of childbirth and self-efficacy of primiparas. Methods Eighty-two primiparas who underwent regular prenatal examination, registration, and delivery at Renji Hospital,Shanghai Jiao Tong University School of Medicine from October 2020 to December 2022 were selected as the research subjects. They were allocated into an observation group and a control group using a random number table, with 41 primiparas in each group. The control group adopted routine obstetric nursing methods, while the observation group adopted nursing methods based on midwife-led counseling. The indicators of fear of childbirth,self-efficacy,postpartum blood loss amount, and adverse delivery events were compared between the two groups. Results After delivery, the Childbirth Attitudes Questionnaire(CAQ)scores of both groups were lower than those before nursing and before delivery, and the Efficacy Expectancy-16(EE-16)and Outcome Expectancy-16(OE-16)scores were higher than those before nursing and before delivery(all P<0.05).The scores before delivery were also better than those before nursing(all P<0.05).Before delivery and after delivery, the CAQ scores of the observation group were lower than those of the control group, and the EE-16 and OE-16 scores were higher than those of the control group(all P<0.05).The first, second, and third stages of labor of the observation group were shorter than those of the control group. There was less blood loss at 2 hours postpartum in the observation group. The total incidence of adverse delivery events was lower in the observation group than in the control group(all P<0.05).Conclusion The nursing model based on midwife-led counseling can help alleviate the fear of childbirth in primiparas, improve the self-efficacy of delivery, shorten the stage of delivery, reduce the postpartum hemorrhage, and reduce the incidence of adverse delivery events.
