摘要
目的 采用TOPSIS法对临床专科医疗服务绩效进行综合评价,为高质量发展下公立医院绩效管理研究与实践提供理论支撑.方法 在宁波市某三甲医院"DRGs绩效考核系统"、"不良事件上报平台"、"门诊管理系统"和"医务系统"中,从医疗服务能力、效率、质量与安全3个维度,选取评价指标,根据专家咨询法确定的各指标权重系数,采用TOPSIS法对该院2022年各临床专科医疗服务绩效进行综合评价.结果 2022年该院54个临床科室中,肛肠外科和疼痛科分别在医疗服务能力和医疗服务效率排名第1.在医疗服务质量与安全方面,疼痛科、重症肝病科、重症医学科、肝硬化科、肺二科、急诊创伤、孕妇肝病科、肺一科、免疫肝科、放疗二科、肿瘤内科、老年医学科、放疗一科和全科医学科14个科室表现较好,均无扣分.综合排名前3的科室依次为肛肠科、心血管内科和口腔科.结论 应用TOPSIS法对公立医院高质量发展下临床专科医疗服务绩效进行综合评价具有很好的实用性.
Abstract
Objective To comprehensively evaluate the performance of clinical specialized medical services by the TOPSIS method,and to provide theoretical support for the research and practice of performance management in public hospitals under the high-quality development.Methods In the DRGs Performance Evaluation System,Adverse Event Reporting Platform,Outpatient Management System,and Medical System of a tertiary hospital in Ningbo City,evaluation indicators were selected from the three dimensions of medical service capability,efficiency and quality and safety.Based on the weight coefficients of each indicator determined by the expert consultation method,the TOPSIS method was used to comprehensively evaluate the performance of various clinical specialty medical services in the hospital in 2022.Results Among the 54 clinical departments in 2022,the Department of Colorectal Surgery and the Department of Pain Management performed the best in terms of medical service capacity and efficiency,respectively.In terms of medical service quality and safety,the Pain Department,Critical Liver Disease Department,Intensive Care Medicine Department,Cirrhosis Department,Pulmonary Department 2,Emergency Trauma Department,Maternal Liver Disease Department,Pulmonary Department 1,Immunoliver Department,Radiotherapy Department 2,Oncology Department,Geriatrics Department,Radiotherapy Department 1,and General Medical Department performed well without any deduction of points.The top three departments in the comprehensive ranking were Proctology,Cardiovascular Medicine,and Stomatology.Conclusions The application of TOPSIS method to comprehensively evaluate the performance of clinical specialized medical services under the high-quality development of public hospitals has good practicality.
