目的 分析2014-2023年重庆市巴南区手足口病(hand,foot and mouth disease,HFMD)流行特征,为巴南区HFMD防控措施提供科学依据.方法 通过中国疾病预防控制信息系统搜集2014-2023年巴南区的HFMD资料,采用描述性流行病学分析和卡方检验进行数据分析,数据使用SPSS 26.0、R 4.1.3软件进行处理,以α=0.05为检验水准.结果 2014-2023年重庆市巴南区共计报告HFMD 22826例,年平均发病率为213.99/10万,发病率呈波动下降趋势(x2趋势=148.48,P<0.001);2014-2019年呈偶数年高发、奇数年低发,发病高峰为4至7月、10至11月;2020-2023年奇数年高发、偶数年低发,年内发病呈单峰;全区22个镇/街道均有病例报告,主要集中在城区和城乡结合部;发病人群以散居儿童与幼托儿童为主;发病年龄以0~<4岁为主,以1~<2岁组最高;病原学监测样本738份,阳性578份,EV 71型占15.29%(89/582)、Cox A16型占25.43%(148/582)、其他肠道病毒型占 59.28%(345/582).结论 2014-2023年重庆市巴南区HFMD年平均发病率呈波动式下降,受新冠疫情影响,HFMD发病规律发生了改变,在2023年反弹,出现新发病高峰,应当重点关注城市城乡结合部等地区及0~<4岁儿童.目前巴南区HFMD病原为Cox A16与其他肠道病毒交替出现,以其他肠道病毒为主.
Epidemic and Etiological Characteristics of HFMD in Banan District of Chongqing from 2014 to 2023
Objective To analyze the epidemic characteristics of hand,foot and mouth disease(HFMD)in Banan District of Chongqing from 2014 to 2023,and to provide a scientific basis for the prevention and control measures of HFMD in Banan district.Methods The HFMD data from Chinese Disease Prevention and Control Information System were collected from 2014 to 2023,descriptive epidemiological analysis and Chi-square test were used to analyze the data.The data was processed by SPSS 26.0 and R 4.1.3 software,the test level α=0.05.Results From 2014 to 2023,a total of HFMD 22 826 cases were reported in Banan District of Chongqing,with the average annual incidence of 213.99/105,and the overall decreasing trend(x2Trend=148.48,P<0.001);From 2014 to 2019,the incidence was higher in even-years and lower in odd-year,the incidence peak was from April to July and from October to November;From 2020 to 2023,the incidence was higher in odd-years and lower in even-years.The incidence was single-peaked within the year.Cases had been reported in 22 towns/streets throughout the district,mainly reported in urban areas and urban fringe,the majority of the patients were scattered children and nursery children;The age of onset was mainly 0-4 years old,the highest group from 1 to 2 years old;738 pathogenic surveillance samples,578 were positive,EV 71 accounted for 15.29%(89/582),Cox A16 accounted for 25.43%(148/582)and other enterovirus types accounted for 59.28%(345/582).Conclusions From 2014 to 2023,the annual average incidence of HFMD in Banan District of Chongqing is on the fluctuating decline.Due to the influence of COVID-19,the incidence of HFMD has changed.In 2023,there was a new peak of incidence,and the focus should be placed on urban and rural fringe areas and children aged 0-4 years.At present,the pathogen of HFMD in Banan district is Cox A16,alternating with other enteric viruses,mainly by other enteric viruses.
