目的 探讨成人不同程度角膜散光对视网膜周边离焦(RDV)的影响.方法 收集近视及顺规散光患者246 例(仅左眼),按散光度数分为 A 组(0<CYL≤-1.50 D)、B组(-1.50<CYL≤-3.00D)和 C 组(CYL>-3.00D),测量眼部生物参数和视网膜各区域RDV,分析各组RDV的分布趋势及差异.结果 三组的近视离焦均在20°内;RDV在水平径向大于垂直径向(P<0.05);各象限RDV的大小排序为:下<上<颞<鼻.Spearman分析显示,CYL与0°~53°的鼻侧RDV均呈正相关(均P<0.05);与0°~20°的颞侧RDV均呈负相关(均P<0.001);与30°~53°的颞侧RDV均呈正相关(均P<0.05).结论 视网膜周边离焦分布不对称,水平径向大于垂直径向;角膜散光对水平径向的周边离焦有影响.
Abstract
Objective To investigate the effect of different degrees of corneal astigmatism on retinal peripheral defo-cus(RDV)in adults.Methods 246 patients with myopia and with-the-rule astigmatism were selected.According to the degree of astigmatism,they were divided into Group A(0<CYL≤-1.50 D),Group B(-1.50<CYL≤-3.00 D),Group C(CYL>-3.00 D).All biological parameters of eyes and RDV in various ranges of retina were examined to ana-lyze the distribution trend and differences of RDV in each group.Results The myopic defocus of all study subjects was in 20° eccentricity range,and the RDV of retina in the horizontal direction was larger than that in the vertical(P<0.05).Quadrant partition,the magnitude of RDV was ranked as follows:inferior<superior<temporal<nasal.Spearman analy-sis showed corneal astigmatism was positively correlated with nasal RDV in 0°-53°(all P<0.05),negatively correlated with temporal RDV in 0°-20°(all P<0.001),and positively correlated with temporal RDV in 30°-53°(all P<0.05).Conclusion The distribution of RDV was asymmetrical,with the horizontal is greater than the vertical.Corneal astigma-tism have an effect on RDV in the horizontal direction.
