超声造影技术对颈动脉硬化病变的诊断价值

摘要
目的:探讨应用超声造影技术对颈动脉粥样硬化病变的诊断价值。方法应用超声造影观察颈动脉粥样硬化患者80例,分别观察造影剂增强及血管充盈情况,并与数字减影血管造影(DSA)对照。超声造影观察斑块内增强并将斑块增强分为0~3级。结果颈动脉超声造影可清晰显示动脉内-中膜厚度,发现37个造影前未发现的斑块,勾勒出动脉斑块大小及形态。与 DSA 对照,应用彩色多普勒超声(CDFI)、超声造影诊断颈动脉粥样硬化所致颈部动脉闭塞的敏感性分别为72.5%、80.5%,特异性分别为90.8%、96.2%,准确率分别为89.6%、93.5%。结论超声造影提高了颈动脉斑块的显示率,有助于颈动脉狭窄程度判断,另外,通过超声造影增强分级可有效评估颈动脉斑块稳定性。
Abstract
Objective To evaluate the diagnostic value of contrast-enhanced ultrasound in carotid atherosclerosis. Methods Eighty cases were examined with contrast-enhanced ultrasonography. The enhancement and blood vessel filling were observed after the injection of the agent,and the results were compared with DSA diagnosis. Enhancement within the plaque was categorized as grade 0 to 3 by visual interpretation and was evaluated by quantitative analysis. Results Thirty-seven plaques were found and the intima-media thickness(IMT)became more clear after injection of the contrast agent. The morphology and structure of plaques was observed at the same time. The sensitivity of CDFI and CEUS compared with DSA in detection of occlusion was 72. 5% ,80. 5% ,the specificity was 90. 8% ,96. 2%and the accuracy was 89. 6% and 93. 5% respectively. Conclusions Contrast-enhanced ultrasound (CEUS)can improve the detection of the arterial plaque and determine the extent of artery stenosis. In addition,contrast-enhanced ultrasound can further assess the stability of carotid plaque.
