Construction of nomogram prediction model for oral mucosal pressure injury in patients after extracorporeal circulation in ICU
Objective To screen the risk factors of oral mucosal pressure injury(OMPI)in ICU patients after extracorporeal circulation(CPB),and establish a nomogram prediction model.Methods 455 patients with extra-corporeal circulation in the ICU of a top three hospital in Xinjiang from January to June 2023 were selected as the study objects.According to whether OMPI occurred,they were divided into the occurrence group and the non-occurrence group.General data,clinical data,surgical data and tracheal catheter data of the two groups were compared.Logistic regression analysis was used to screen the risk factors of OMPI in ICU patients after cardiop-ulmonary bypass,and the prediction model of nomogram was constructed and verified internally.Results Among the 455 patients,135(29.7%)developed OMPI.Logistic regression analysis showed that the history of hyper-tension(OR=9.529,95%CI:2.685-33.089),postoperative ECMO(OR=3.251,95%CI:1.380-7.662),preoperative hematocrit(40%-50%OR=5.755,95%CI:1.703-19.453),and preoperative fast-ing blood glucose(7.1-11.0 mmol/L,OR=2.415,95%CI:1.717-4.975;>11.0 mmol/L,OR=3.401,95%CI:1.571-7.361)were independent risk factors(P<0.05).The minimum intraoperative body tempera-ture(OR=0.283,95%CI:0.214-0.374)was a protective factor(P<0.05).The nomogram prediction model based on the above influencing factors shows that AUC=0.883,sensitivity is 86.7%,and specificity is 77.2%.H-L test shows that:x2=3.848,P=0.871,the column-line prediction model has a good degree of dif-ferentiation and calibration.The calibration curve shows that the probability predicted by the model is in good a-greement with the probability formed by the actual observation.Conclusion The nomogram prediction model of OMPI after cardiopulmonary bypass established in this study has good risk prediction ability,and has practical guiding significance for clinical work.
