The association between quality of life,anxiety,and depression in elderly in-appropriate medication user
Objective To explore the association between quality of life,anxiety,and depression in elderly in-appropriate medication users,and to provide reference for early intervention.Methods From May 2017 to May 2018,we investigated the elderly populations over the age of 65 years in the primary healthcare stations of Bei-jing Shijitan Hospital Alliance.Demographic characteristics,chronic diseases,medication use,SF-36 quality of life score,hospital anxiety and depression score(HADS),and other factors were collected in the survey.Statis-tical description of general data,the multivariate logistic regression model was used to analyze the relationship between quality of life,anxiety,and depression,in elderly inappropriate medication users.Results The inappro-priate medication use rate was 42.4%(386/977)in this study,and the proportion of patients with high scores of anxiety,depression,and charlson comorbidity index was higher than rational drug users.In the physical func-tioning,bodily pain,social functioning,and role-emotional of SF-36 scores of elderly inappropriate medication users(excluding role-physical,general health,vitality,and mental health)were lower than those of appropriate medication users,while the Hospital Anxiety Scale and Hospital Depression Scale scores were higher than those of appropriate medication users(all P<0.05).Multivariate logistic regression model analysis showed that:anx-iety and depression in elderly inappropriate medication users were associated with low scores of Bodily Pain(OR=1.842,95%CI:1.020-3.326;OR=1.255,95%CI:1.076-2.333),Vitality(OR=4.310,95%CI:1.018-18.248;OR=19.453,95%CI:3.633-34.180),Role-Emotional(OR=1.678,95%CI:1.064-3.682;OR=4.348,95%CI:1.929-9.798),Mental Health(OR=2.009,95%CI:1.799-5.052;OR=4.761,95%CI:1.798-12.606)in the SF-36 score,and the higher the CCI score,the higher the risk of anx-iety and depression,showing a significant dose-response relationship(all P<0.05);However,no correlation was found among elderly appropriate medication users with anxiety and depression,except the CCI score and the Mental Health of SF-36(all P<0.05).Conclusion The problem of inappropriate medication use in the elderly is very common,and their quality of life is reduced to varying degrees.Poor quality of life is closely related to anxiety and depression in elderly inappropriate medication users.
