Analysis of drug resistance characteristics of pulmonary tuberculosis in Zunyi city and influential factors of rifampicin resistance
Objective Drug-resistant tuberculosis(DR-TB)is an important obstacle to achieving the goal of ending the global TB epidemic by 2035.China is a country with high prevalence of multidrag-resistant tubercu-losis(MDR-TB)and Rifampicin-resistant tuberculosis(RR-TB)in which Guizhou province is one of the regions with a serious tuberculosis epidemic.This study analyzed the epidemiological characteristics of drug-resistant tu-berculosis in Zunyi City to explore its epidemic trend.Methods Clinical information and sputum or bronchoalve-olar lavage fluid samples of tuberculosis patients from the Affiliated Hospital of Zunyi Medical University from 2016 to 2018 were collected.Mycobacterium tuberculosis was isolated and cultured using Lowenstein-Jensen(L-J)medium,and its sensitivity to four first-line and six second-line anti-tuberculosis drugs was detected by tradi-tional drug sensitivity test.The prevalence trend of drug-resistant TB from 2008 to 2018 was retrospectively com-pared,and the related factors of rifampicin resistance were analyzed.Results Among the 1 063 patients with tu-berculosis,the incidence of male patients was significantly higher than that of female patients,with the age group of mainly covered 16-79 years old.Rifampicin-resistant tuberculosis(RR-TB)accounted for 13.2%(140/1 063),and Multidrug-resistant tuberculosis(MDR-TB)accounted for 11.5%(122/1 063).Pre-exten-sive drug resistance tuberculosis(pre-XDR-TB)accounted for 15.1%(37/245)of randomized second-line drug-sensitive cases.Age group,treatment history and regional distribution were the factors influencing rifampi-cin resistance.Conclusion Although the epidemic situation of drug-resistant TB in Guizhou Province has a remis-sion trend,the incidence of MDR/RR-TB is still at a high level,and the level of surveillance and hierarchical diagnosis and treatment of drug-resistant TB in Guizhou province should be strengthened.
