摘要

糖尿病肾脏病(DKD)是糖尿病(DM)最为常见的并发症之一,可进展至终末期肾脏病,严重降低患者的生活质量.线粒体自噬通过自噬机制选择性地清除受损或功能失调的线粒体,以维持线粒体质量控制和体内平衡,是自噬现象的一种.研究表明,线粒体自噬在DKD的发生、发展过程中发挥重要作用,基于线粒体自噬角度防治DKD是现阶段的研究热点之一.中医药具有全方位、多靶点的治疗优势,笔者基于线粒体自噬途径及相关信号通路的调节对中医药治疗DKD的研究进展进行梳理,发现部分补益药、活血药、利水渗湿药、清热解毒药等中药单体或复方,可通过PINK1/Parkin、FUNDC1、AMPK/SIRT1等信号通路调控线粒体自噬,进而有效延缓DKD的进展.

Abstract

Diabetic kidney disease(DKD)is one of the most common complications of diabetes mellitus(DM)and can progress to end-stage renal disease,significantly reducing patients' quality of life.Mitochondrial autophagy selectively clears damaged or dysfunctional mitochondria through autophagy mechanisms to maintain mitochondrial quality control and homeostasis,representing a form of autophagy.Studies have shown that mitochondrial autophagy plays a crucial role in the occurrence and development of DKD.Therefore,preventing and treating DKD from the perspective of mitochondrial autophagy has become a research hotspot.Traditional Chinese Medicine(TCM)has comprehensive,multi-targeted therapeutic advantages.This paper reviews the research progress on TCM treatment of DKD through the regulation of mitochondrial autophagy pathways and related signaling pathways.It is found that some tonifying medicines,blood-activating medicines,diuretic dampness-percolating medicines,and heat-clearing detoxifying medicines,either as monomers or compounds,can regulate mitochondrial autophagy through PINK1/Parkin,FUNDC1,AMPK/SIRT1,and other signaling pathways,thereby effectively delaying the progression of DKD.
