摘要

目的:探究海藻玉壶汤合透脓散加减联合火针疗法对痰瘀凝结型重度痤疮患者皮肤生理功能的影响.方法:选取符合纳入标准的痰瘀凝结型重度痤疮患者126例,依据随机数字表随机分为研究组和对照组,每组各63例.对照组采用常规西药治疗,研究组采用海藻玉壶汤合透脓散加减联合火针疗法,两组均治疗8周.比较两组患者治疗前后皮肤生理功能(TEWL、表皮含水量、油脂指数)、免疫功能(CD3+、CD4+、CD8+)、中医症状评分(皮损形态、性质、颜色变化评分和皮损计数评分、同一部位疗前疗后皮肤相片对比评分、全身症及舌象脉象变化评分)的变化情况,并比较临床疗效.结果:研究组临床总有效率为96.83%(61/63),对照组临床总有效率84.13%(53/63),两组比较差异有统计学意义(P<0.05).与治疗前比较,两组患者治疗后TEWL水平降低(P<0.05),表皮含水量、油脂指数升高(P<0.05);与对照组治疗后比较,研究组TEWL水平降低(P<0.01),表皮含水量、油脂指数升高(P<0.05).与治疗前比较,研究组治疗后CD3+、CD8+水平降低(P<0.05),CD4+水平升高(P<0.05);与对照组治疗后比较,研究组CD3+、CD8+水平降低(P<0.05,P<0.01),CD4+水平升高(P<0.01).与治疗前比较,两组患者治疗后中医症状评分均降低(P<0.05);与对照组治疗后比较,研究组中医症状评分均明显降低(P<0.01).结论:海藻玉壶汤合透脓散加减联合火针疗法能显著改善痰瘀凝结型重度痤疮患者的皮肤生理功能、免疫功能,中医临床症状明显改善.

Abstract

Objective:To explore the effects of modified Haizao Yuhu Decoction and Tounong Powder combined with fire needle therapy on skin physiological function in patients with phlegm-stasis congealing type severe acne.Methods:A total of 126 patients with phlegm-stasis congealing type severe acne meeting the inclusion criteria were randomly divided into a study group and a control group,with 63 patients in each group.The control group received conventional Western medicine treatment,while the study group received modified Haizao Yuhu Decoction and Tounong Powder combined with fire needle therapy.Both groups were treated for 8 weeks.The skin physiological functions(TEWL,epidermal moisture content,and sebum index),immune function(CD3+,CD4+,CD8+),and traditional Chinese medicine(TCM)symptom scores were measured before and after treatment,and clinical efficacy was compared between the two groups.Results:The total effective rate in the study group was 96.83%(61/63),significantly higher than 84.13%(53/63)in the control group(P<0.05).Post-treatment,TEWL levels in both groups decreased(P<0.05),while epidermal moisture content and sebum index increased(P<0.05).The study group showed a greater reduction in TEWL levels(P<0.01)and higher increases in epidermal moisture content and sebum index(P<0.05)compared to the control group.For immune function,CD3+and CD8+levels decreased(P<0.05)while CD4+levels increased(P<0.05)in the study group after treatment.When compared to the control group,the study group showed significantly lower levels of CD3+and CD8+(P<0.05,P<0.01)and higher CD4+levels(P<0.01).TCM symptom scores decreased significantly in both groups(P<0.05),with the study group showing a more significant reduction(P<0.01).Conclusion:The combined treatment of modified Haizao Yuhu Decoction and Tounong Powder combined with fire needle therapy significantly improves skin physiological function and immune function in patients with phlegm-stasis congealing type severe acne,with notable improvements in TCM clinical symptoms.
