    Expert consensus on odontogenic maxillary sinusitis multi-disciplinary treatment

    查看更多>>摘要:Odontogenic maxillary sinusitis(OMS)is a subtype of maxillary sinusitis(MS).It is actually inflammation of the maxillary sinus that secondary to adjacent infectious maxillary dental lesion.Due to the lack of unique clinical features,OMS is difficult to distinguish from other types of rhinosinusitis.Besides,the characteristic infectious pathogeny of OMS makes it is resistant to conventional therapies of rhinosinusitis.Its current diagnosis and treatment are thus facing great difficulties.The multi-disciplinary cooperation between otolaryngologists and dentists is absolutely urgent to settle these questions and to acquire standardized diagnostic and treatment regimen for OMS.However,this disease has actually received little attention and has been underrepresented by relatively low publication volume and quality.Based on systematically reviewed literature and practical experiences of expert members,our consensus focuses on characteristics,symptoms,classification and diagnosis of OMS,and further put forward multi-disciplinary treatment decisions for OMS,as well as the common treatment complications and relative managements.This consensus aims to increase attention to OMS,and optimize the clinical diagnosis and decision-making of OMS,which finally provides evidence-based options for OMS clinical management.

    查看更多>>摘要:Endodontic diseases are a kind of chronic infectious oral disease.Common endodontic treatment concepts are based on the removal of inflamed or necrotic pulp tissue and the replacement by gutta-percha.However,it is very essential for endodontic treatment to debride the root canal system and prevent the root canal system from bacterial reinfection after root canal therapy(RCT).Recent research,encompassing bacterial etiology and advanced imaging techniques,contributes to our understanding of the root canal system's anatomy intricacies and the technique sensitivity of RCT.Success in RCT hinges on factors like patients,infection severity,root canal anatomy,and treatment techniques.Therefore,improving disease management is a key issue to combat endodontic diseases and cure periapical lesions.The clinical difficulty assessment system of RCT is established based on patient conditions,tooth conditions,root canal configuration,and root canal needing retreatment,and emphasizes pre-treatment risk assessment for optimal outcomes.The findings suggest that the presence of risk factors may correlate with the challenge of achieving the high standard required for RCT.These insights contribute not only to improve education but also aid practitioners in treatment planning and referral decision-making within the field of endodontics.

    查看更多>>摘要:Chemical cleaning and disinfection are crucial steps for eliminating infection in root canal treatment.However,irrigant selection or irrigation procedures are far from clear.The vapor lock effect in the apical region has yet to be solved,impeding irrigation efficacy and resulting in residual infections and compromised treatment outcomes.Additionally,ambiguous clinical indications for root canal medication and non-standardized dressing protocols must be clarified.Inappropriate intracanal medication may present side effects and jeopardize the therapeutic outcomes.Indeed,clinicians have been aware of these concerns for years.Based on the current evidence of studies,this article reviews the properties of various irrigants and intracanal medicaments and elucidates their effectiveness and interactions.The evolution of different kinetic irrigation methods,their effects,limitations,the paradigm shift,current indications,and effective operational procedures regarding intracanal medication are also discussed.This expert consensus aims to establish the clinical operation guidelines for root canal irrigation and a position statement on intracanal medication,thus facilitating a better understanding of infection control,standardizing clinical practice,and ultimately improving the success of endodontic therapy.

    查看更多>>摘要:Oral diseases,such as periodontitis,salivary gland diseases,and oral cancers,significantly challenge health conditions due to their detrimental effects on patient's digestive functions,pronunciation,and esthetic demands.Delayed diagnosis and non-targeted treatment profoundly influence patients'prognosis and quality of life.The exploration of innovative approaches for early detection and precise treatment represents a promising frontier in oral medicine.Exosomes,which are characterized as nanometer-sized extracellular vesicles,are secreted by virtually all types of cells.As the research continues,the complex roles of these intracellular-derived extracellular vesicles in biological processes have gradually unfolded.Exosomes have attracted attention as valuable diagnostic and therapeutic tools for their ability to transfer abundant biological cargos and their intricate involvement in multiple cellular functions.In this review,we provide an overview of the recent applications of exosomes within the field of oral diseases,focusing on inflammation-related bone diseases and oral squamous cell carcinomas.We characterize the exosome alterations and demonstrate their potential applications as biomarkers for early diagnosis,highlighting their roles as indicators in multiple oral diseases.We also summarize the promising applications of exosomes in targeted therapy and proposed future directions for the use of exosomes in clinical treatment.

    Nitrate reduction capacity of the oral microbiota is impaired in periodontitis:potential implications for systemic nitric oxide availability

    查看更多>>摘要:The reduction of nitrate to nitrite by the oral microbiota has been proposed to be important for oral health and results in nitric oxide formation that can improve cardiometabolic conditions.Studies of bacterial composition in subgingival plaque suggest that nitrate-reducing bacteria are associated with periodontal health,but the impact of periodontitis on nitrate-reducing capacity(NRC)and,therefore,nitric oxide availability has not been evaluated.The current study aimed to evaluate how periodontitis affects the NRC of the oral microbiota.First,16S rRNA sequencing data from five different countries were analyzed,revealing that nitrate-reducing bacteria were significantly lower in subgingival plaque of periodontitis patients compared with healthy individuals(P<0.05 in all five datasets with n=20-82 samples per dataset).Secondly,subgingival plaque,saliva,and plasma samples were obtained from 42 periodontitis patients before and after periodontal treatment.The oral NRC was determined in vitro by incubating saliva with 8 mmol/L nitrate(a concentration found in saliva after nitrate-rich vegetable intake)and compared with the NRC of 15 healthy individuals.Salivary NRC was found to be diminished in periodontal patients before treatment(P<0.05)but recovered to healthy levels 90 days post-treatment.Additionally,the subgingival levels of nitrate-reducing bacteria increased after treatment and correlated negatively with periodontitis-associated bacteria(P<0.01).No significant effect of periodontal treatment on the baseline saliva and plasma nitrate and nitrite levels was found,indicating that differences in the NRC may only be revealed after nitrate intake.Our results suggest that an impaired NRC in periodontitis could limit dietary nitrate-derived nitric oxide levels,and the effect on systemic health should be explored in future studies.

    查看更多>>摘要:Bone formation and deposition are initiated by sensory nerve infiltration in adaptive bone remodeling.Here,we focused on the role of Semaphorin 3A(Sema3A),expressed by sensory nerves,in mechanical loads-induced bone formation and nerve withdrawal using orthodontic tooth movement(OTM)model.Firstly,bone formation was activated after the 3rd day of OTM,coinciding with a decrease in sensory nerves and an increase in pain threshold.Sema3A,rather than nerve growth factor(NGF),highly expressed in both trigeminal ganglion and the axons of periodontal ligament following the 3rd day of OTM.Moreover,in vitro mechanical loads upregulated Sema3A in neurons instead of in human periodontal ligament cells(hPDLCs)within 24 hours.Furthermore,exogenous Sema3A restored the suppressed alveolar bone formation and the osteogenic differentiation of hPDLCs induced by mechanical overload.Mechanistically,Sema3A prevented overstretching of F-actin induced by mechanical overload through ROCK2 pathway,maintaining mitochondrial dynamics as mitochondrial fusion.Therefore,Sema3A exhibits dual therapeutic effects in mechanical loads-induced bone formation,both as a pain-sensitive analgesic and a positive regulator for bone formation.

    RNPS1 stabilizes NAT10 protein to facilitate translation in cancer via tRNA ac4C modification

    查看更多>>摘要:Existing studies have underscored the pivotal role of N-acetyltransferase 10(NAT10)in various cancers.However,the outcomes of protein-protein interactions between NAT10 and its protein partners in head and neck squamous cell carcinoma(HNSCC)remain unexplored.In this study,we identified a significant upregulation of RNA-binding protein with serine-rich domain 1(RNPS1)in HNSCC,where RNPS1 inhibits the ubiquitination degradation of NAT10 by E3 ubiquitin ligase,zinc finger SWIM domain-containing protein 6(ZSWIM6),through direct protein interaction,thereby promoting high NAT10 expression in HNSCC.This upregulated NAT10 stability mediates the enhancement of specific tRNA ac4C modifications,subsequently boosting the translation process of genes involved in pathways such as IL-6 signaling,IL-8 signaling,and PTEN signaling that play roles in regulating HNSCC malignant progression,ultimately influencing the survival and prognosis of HNSCC patients.Additionally,we pioneered the development of TRMC-seq,leading to the discovery of novel tRNA-ac4C modification sites,thereby providing a potent sequencing tool for tRNA-ac4C research.Our findings expand the repertoire of tRNA ac4C modifications and identify a role of tRNA ac4C in the regulation of mRNA translation in HNSCC.

    WNT7A promotes tumorigenesis of head and neck squamous cell carcinoma via activating FZD7/JAK1/STAT3 signaling

    查看更多>>摘要:Wnt signaling are critical pathway involved in organ development,tumorigenesis,and cancer progression.WNT7A,a member of the Wnt family,remains poorly understood in terms of its role and the underlying molecular mechanisms it entails in head and neck squamous cell carcinoma(HNSCC).According to the Cancer Genome Atlas(TCGA),transcriptome sequencing data of HNSCC,the expression level of WNT7A in tumors was found to be higher than in adjacent normal tissues,which was validated using Real-time RT-PCR and immunohistochemistry.Unexpectedly,overexpression of WNT7A did not activate the canonical Wnt-β-catenin pathway in HNSCC.Instead,our findings suggested that WNT7A potentially activated the FZD7/JAK1/STAT3 signaling pathway,leading to enhanced cell proliferation,self-renewal,and resistance to apoptosis.Furthermore,in a patient-derived xenograft(PDX)tumor model,high expression of WNT7A and phosphorylated STAT3 was observed,which positively correlated with tumor progression.These findings underscore the significance of WNT7A in HNSCC progression and propose the targeting of key molecules within the FZD7/JAK1/STAT3 pathway as a promising strategy for precise treatment of HNSCC.

    A novel saliva-based miRNA profile to diagnose and predict oral cancer

    查看更多>>摘要:Oral cancer(OC)is the most common form of head and neck cancer.Despite the high incidence and unfavourable patient outcomes,currently,there are no biomarkers for the early detection of OC.This study aims to discover,develop,and validate a novel saliva-based microRNA signature for early diagnosis and prediction of OC risk in oral potentially malignant disorders(OPMD).The Cancer Genome Atlas(TCGA)miRNA sequencing data and small RNA sequencing data of saliva samples were used to discover differentially expressed miRNAs.Identified miRNAs were validated in saliva samples of OC(n=50),OPMD(n=52),and controls(n=60)using quantitative real-time PCR.Eight differentially expressed miRNAs(miR-7-5p,miR-10b-5p,miR-182-5p,miR-215-5p,miR-431-5p,miR-486-3p,miR-3614-5p,and miR-4707-3p)were identified in the discovery phase and were validated.The efficiency of our eight-miRNA signature to discriminate OC and controls was:area under curve(AUC):0.954,sensitivity:86%,specificity:90%,positive predictive value(PPV):87.8%and negative predictive value(NPV):88.5%whereas between OC and OPMD was:AUC:0.911,sensitivity:90%,specificity:82.7%,PPV:74.2%and NPV:89.6%.We have developed a risk probability score to predict the presence or risk of OC in OPMD patients.We established a salivary miRNA signature that can aid in diagnosing and predicting OC,revolutionising the management of patients with OPMD.Together,our results shed new light on the management of OC by salivary miRNAs to the clinical utility of using miRNAs derived from saliva samples.

    Spatial transcriptomics reveals that metabolic characteristics define the tumor immunosuppression microenvironment via iCAF transformation in oral squamous cell carcinoma

    查看更多>>摘要:Tumor progression is closely related to tumor tissue metabolism and reshaping of the microenvironment.Oral squamous cell carcinoma(OSCC),a representative hypoxic tumor,has a heterogeneous internal metabolic environment.To clarify the relationship between different metabolic regions and the tumor immune microenvironment(TME)in OSCC,Single cell(SC)and spatial transcriptomics(ST)sequencing of OSCC tissues were performed.The proportion of TME in the ST data was obtained through SPOTlight deconvolution using SC and GSE103322 data.The metabolic activity of each spot was calculated using scMetabolism,and k-means clustering was used to classify all spots into hyper-,normal-,or hypometabolic regions.CD4T cell infiltration and TGF-β expression is higher in the hypermetabolic regions than in the others.Through CellPhoneDB and NicheNet cell-cell communication analysis,it was found that in the hypermetabolic region,fibroblasts can utilize the lactate produced by glycolysis of epithelial cells to transform into inflammatory cancer-associated fibroblasts(iCAFs),and the increased expression of HIF1A in iCAFs promotes the transcriptional expression of CXCL12.The secretion of CXCL12 recruits regulatory T cells(Tregs),leading to Treg infiltration and increased TGF-β secretion in the microenvironment and promotes the formation of a tumor immunosuppressive microenvironment.This study delineates the coordinate work axis of epithelial cells-iCAFs-Tregs in OSCC using SC,ST and TCGA bulk data,and highlights potential targets for therapy.
