    Evaluating Early Nerve Injury and Its Relationship With Leprosy Reactions in Patients With Leprosy:A Prospective Cohort Study

    查看更多>>摘要:Objective:Leprosy is an infectious disease that can be accompanied by disability caused by peripheral neuropathy.Early detection of leprosy is essential to reduce its debilitating sequelae.Ultrasound is a noninvasive test that can detect nerve loss,but ultrasound measurement depends on the skill level of operators.Radiomics is an emerging field related to the extraction and quantification of information,which may provide new solutions for early detection of neuropathy caused by leprosy.This study aimed to combine radiomics and ultrasonography to evaluate nerve damage in the early stages of leprosy in China.Methods:Patients were enrolled at the Wenshan Prefecture Institute of Dermatology between October 2018 and December 2022.High-resolution ultrasound imaging of the peripheral nerves was performed.Quantitative ultrasound features were extracted from the images from 5 follow-ups.Radiomic features were selected for a comparative analysis of the disease course using a linear mixed-effects model to control for potential confounding effects.Results:Nine patients with leprosy were enrolled in this study,consisting of 5 males and 4 females with an average age of 34.44±13.58 years.With the change of treatment time,the ultrasound radiomics features of different parts of the linear mixed-effects model showed a trend of change:the nerve density(mean,skewness)was improved,the fascicular structure(kurtosis)was restored,and the abnormal morphological features in some parts were reduced(P<0.05).Patients with type 1 leprosy reactions exhibited a lower mean and skewness,higher kurtosis,and increased quantitative morphological characteristics.Conclusion:The study provided the feasibility of ultrasound radiomics to digitally visualize the progression of nerve damage in leprosy patients.Ultrasound radiomics can be a low-cost method with high-throughput characteristics for clinical operators of different skill levels in early nerve injury detection of leprosy.

    Reflectance Confocal Microscopy and Dermoscopic Imaging Features in Extramammary Paget's Disease:A Retrospective Study

    查看更多>>摘要:Objective:Extramammary Paget's disease(EMPD)is often misdiagnosed as chronic dermatitis or eczema of the scrotum because of its clinical manifestations,leading to delayed diagnosis and treatment.Reflectance confocal microscopy(RCM)and dermoscopy are widely used in clinical practice,enabling early diagnosis and treatment assessment of skin diseases,particularly which are easily misdiagnosed.This study aimed to summarize the RCM and dermoscopic features of EMPD which may provide evidence for diagnosis and evaluation of treatment effects.Methods:A retrospective analysis was conducted on skin lesions from 34 patients diagnosed with EMPD,confirmed through histopathological examination at the dermatology departments of Chengdu Second People's Hospital and the Japan-China Friendship Hospital between January 2022 and December 2023.The analysis focused on typical changes of lesions,such as blood vessel patterns and background color under dermoscopy,as well as cellular morphology under RCM.Count data were presented as constituent ratios.Results:Among the included 34 patients,28 were male and 6 were female,and 104 skin lesions were identified.The lesions were classified into 3 types:36 red patches,41 dark red erosive patches,and 27 brown pigmented patches.Dermoscopic examination of EMPD revealed characteristic features,including milky-white globules(90.4%,94/104),irregular shiny white streaks(91.3%,95/104),dotted blood vessels(91.3%,95/104),and glomerular-like vessels(88.5%,92/104).RCM demonstrated epidermal structural disorganization(100.0%,104/104),vacuolated cells in the epidermis(94.2%,98/104),and Paget cells,either scattered or clustered(88.5%,92/104).The red macular lesions predominantly contained isolated vacuolated cells in the epidermis,the dark red erosive lesions exhibited Paget cell nests in the basal layer,and the brown pigmented lesions contained numerous dendritic cells.All the patients underwent wide surgical excision followed by RCM and dermoscopic examinations postoperatively.Conclusion:This study identified characteristic dermoscopic and RCM features in patients with EMPD,providing a foundation for noninvasive auxiliary diagnosis and guiding postoperative follow-up.Additionally,RCM characteristics of different lesion morphologies can help physicians more accurately assess surgical margins,leading to more precise surgical treatment options.

    Clinical Application of CLSM,Dermoscope,and Wood's Lamp in Diagnosis and Differential Diagnosis of Five Hypopigmentation Disorders:A Cross-Sectional Study

    查看更多>>摘要:Objective:Noninvasive instruments including confocal laser scanning microscope(CLSM),dermoscope,and Wood's lamp afford the advantage of obtaining skin microanatomical images quickly,in real-time,and dynamically.However,further understanding is needed regarding the differences between various techniques for diagnosis on hypopigmentation diseases.This study was aimed to observe the characteristics of hypopigmentation diseases by using CLSM,dermoscope,and Wood's lamp,in order to provide evidence for clinician to choose appropriate instruments for assistant diagnosis.Methods:In total,data from 397 patients with vitiligo,nevus anemicus,achromic nevus,pityriasis alba,and senile leukoderma were collected from January 2019 to January 2021 in affiliated hospital of Xuzhou Medical University.CLSM,dermoscope,and Wood's lamp were used to analyze the characteristics of lesions and the differences between the diseases.Enumeration data were presented as frequencies and percentages.Results:CLSM showed that the pigment ring of vitiligo was absent(126/126,100.0%),senile leukoderma had hypopigmentation,skin atrophy and thinning,and flat epidermis mutation(79/79,100.0%).In dermoscope,vitiligo showed a milky white hypopigmented area(113/126,89.8%),nevus anemicus showed a clear but irregular(38/45,84.4%)white hypopigmentation area(41/45,91.1%),pityriasis alba showed a clear and light white hypopigmented area,with a grayish-white chaff-like scaly appearance(98/107,91.6%),senile leukoderma showed a white radially hypopigmentation area with a small amount of pigmentation(79/79,100.0%).Under Wood's lamp,bright blue-white patches with clear boundaries can be observed in vitiligo,nevus anemicus,pityriasis alba(93/107,86.9%)and senile leukoderma appeared clear bluish-white patches,achromic nevus appeared unclear bluish-white patches,respectively.Conclusion:Vitiligo,nevus anemicus,achromic nevus,pityriasis alba,and senile leukoderma all belong to hypopigmentation diseases,however,they have own imaging characteristics under CLSM,dermoscope,and Woods lamp techniques,which may be used to help the diagnosis and differential diagnosis of these diseases.

    Uniform Granular Pigmentation as a Novel Dermoscopic Feature of Drug Reaction:A Cross-Sectional Study

    查看更多>>摘要:Objective:In clinical practice,black-brown-gray pigment granules are common under dermoscopy.However,little is known about their clinical significance.We conducted this study to investigate the relationship between dermoscopy features and their representative pathological features.Methods:A cross-sectional study was conducted by searching clinical database in Xiangya Hospital,Central South University,from August 2017 to August 2022.Patients with full information on pigment granules and demographic and pathological findings were entered.Cases were reviewed for information regarding pathological and dermoscopic patterns.Fisher exact test was used to examine the correlation between individual histologic variables and established diagnoses.The depth of pigment granules showed a nonnormal distribution and was analyzed using a nonparametric test.Results:In total,data from 83 patients were included in this analysis.The presence of pigment granules under dermoscopy significantly indicated destruction of basal cells or pigment incontinence(91.5%,76/83).The pigment granules within lesions were divided into uniform and nonuniform patterns with respect to their size,color,and distribution.Uniform granular pigmentation was more likely to be associated with eosinophils(numerous or in aggregates,P<0.001)or perifollicular inflammatory infiltration in the dermis(P=0.008).Notably,brown pigment granules(P=0.024)and uniform granular pigmentation(P<0.001)were more likely to be induced by medications.Conclusion:Uniform granular pigmentation under dermoscopy is a novel indicator of a drug reaction and necessitates a detailed inquiry.

    查看更多>>摘要:Objective:The success of managing atopic dermatitis(AD)largely depends on patient adherence to treatment.Achieving adherence requires more than knowledge transfer;it also requires a change in patients'health behavior Therefore,we conducted this study to develop and validate the content of health games for the pediatric population with AD.Methods:The study was conducted from August 2019 to February 2024 in the Hospital Complex of the Federal University of Paraná,Curitiba,Paraná,Brazil.The content of health games about AD were the book Oli e o Grande Desafio da CoceiraTM("Oli and the Great Itching Challenge")and the games DermatrilhaTM("Dermatrail")and Caixinha Surpresa da DermatiteTM("Dermatitis Surprise Box").The instrument development process consisted of 5 stages:planning of the instruments,development of the instruments,development of content validation questionnaires,evaluation by an expert committee comprising 5 pediatric dermatologists,5 hospital psychologists,5 illustrators/designers,and 5 early childhood education teachers,and a pre-test with the target audience comprising 25 children with AD aged 5 to 12 years.The content validity index was used to assess content validity.Results:The content validity index was 0.94 in the evaluation by the expert committee and 0.95 in the evaluation by the target audience.Among the games,the general content validity index was 0.95 among the members of the expert committee and 0.92 among the target audience,being higher than 0.80 for all items.Conclusion:The health games evaluated in this study are useful psychoeducational tools with high content validity for use in the therapeutic education of children with AD.

    查看更多>>摘要:Objective:Sexually transmitted diseases are infections transmitted from an infected person to an uninfected person through sexual contact.We conducted this study to evaluate the prevalence and the risk factors of coinfection because there has been few research that thoroughly demonstrates its occurrence in Indonesia,particularly in Surabaya.Methods:This research is a retrospective analytical study at Dr Soetomo General Academic Hospital in a period of 2019 to 2021,and primarily aimed to assess risk factors associated with coinfection events in patients with sexually transmitted infections.Patient's data included sex,age,occupation,marital status,comorbid factors,number of sexual partners,sexual orientation,and patient's nonsexual behavior were taken from the medical records for analysis.The chi-square test was used to analyze the correlations between variables.Results:There were 416 sexually transmitted infection patients between 2019 and 2021.Among them,187 patients were proved to have coinfections,of which 184 patients had 2 coinfections and 3 patients had 3 coinfections.There was a statistically significant difference in the presence of coinfections according to sex and sexual orientation(both P<0.001),however,there was no statistically significant association of age,patient's occupation,marital status,comorbidity,and sexual partner with coinfection of patients with sexually transmitted infections.Conclusion:The incidence of coinfection is significantly associated with both sex and sexual orientation in Surabaya,Indonesia.The identified risk factors of coinfection should be considered in the development of local public health strategies to prevent and control sexually transmitted diseases.

    Tyrosine Kinase-2 Inhibitor(Deucravacitinib)for Psoriasis:A Systematic Review and Network Meta-Analysis of Randomized Controlled Trials

    查看更多>>摘要:Objective:Deucravacitinib is a tyrosine kinase-2 inhibitor that shows promise as an emerging treatment option for psoriasis.We aimed to evaluate the efficacy and safety of different doses of deucravacitinib vs.placebo or apremilast on psoriasis treatment.Methods:We performed a systematic review and pairwise and network meta-analyses of randomized controlled trials published in Web of Science,SCOPUS,EMBASE,PubMed,and Cochrane Central databases from the inception of databases until March 31,2023.We used the risk ratio for dichotomous outcomes,and the mean difference with corresponding 95%confidence interval for continuous outcomes.We registered our protocol in PROSPERO(ID:CRD42023413976).Results:We analyzed 4 randomized controlled trials involving 2,156 patients randomly assigned to receive placebo,60 mg of apremilast once daily,or various doses of deucravacitinib(3 mg every other day,3 mg once daily,3 mg twice daily,6 mg twice daily,or 12 mg once daily).Deucravacitinib achieved significantly better outcomes compared with placebo across multiple efficacy measures,including the Psoriasis Area and Severity Index score(P<0.01),Static Physician's Global Assessment score(P<0.01),Psoriasis Symptomsand Signs Diary score(P<0.01),Dermatology Life Quality Index score(P<0.01),and scalp-specific Physician's Global Assessment score(P<0.01).When comparing 3 mg of deucravacitinib twice daily with 60 mg of apremilast once daily,deucravacitinib exhibited superior outcomes in terms of the Psoriasis Area and Severity Index(P<0.01),Dermatology Life Quality Index(P<0.01),Static Physician's Global Assessment(P<0.01),and scalp-specific Physician's Global Assessment scores(P<0.01).Deucravacitinib and apremilast were associated with a higher incidence of adverse events compared with placebo,including diarrhea,nausea,headache,and infections.However,no significant differences were observed between interventions regarding serious adverse events or adverse events leading to discontinuation.Conclusion:Deucravacitinib and apremilast show promising efficacy as psoriasis treatments.While adverse events were more common with these treatments compared with placebo,the incidence of serious adverse events did not significantly differ between the intervention and placebo groups.
