    Clinical characteristics of sudden hearing loss during pregnancy

    查看更多>>摘要:Objective:The objective of this study was to explore the clinical characteristics and management of sudden hearing loss(HL)during pregnancy,thus better guiding the clinical practice.Methods:The clinical and follow-up data of 17 patients(17 ears)with sudden HL during pregnancy were analyzed retrospectively(the observe group).Twelve nonpregnant female patients(12 ears)with sudden HL of similar clinical characteristics were selected as the control group.The prognosis of the two groups was compared.All the patients were followed up after delivery,and two of them were readmitted to the hospital 1-2 months after delivery.Results:The observe group had better improvement in hearing and a higher response rate compared to the control group.The pure tone hearing and speech recognition rate of patients could still be improved after the readmitted treatment,and the hearing could partially recover spontaneously during follow-up.The laboratory indicators that affect the inflammatory response and coagulation pathway were significantly different between the two groups.Conclusions:The hearing condition of sudden HL during pregnancy is severe,and the prognosis of these patients is better than nonpregnant patients of similar clinical characteristics.Postpartum treatment is still effective,and some patients showed self-healing with time during follow-up.The inflammatory response and coagulation function may affect the hearing of patients through a metabolic pathway.

    A cross-sectional study on predictors of patients'tinnitus severity

    查看更多>>摘要:Objective:To identify factors that influence the severity of tinnitus via a hierarchical multiple linear regression model.Methods:The study was a retrospective cross-sectional analysis.The study included 331 patients experiencing tinnitus as their primary concern,who visited Shanghai Changzheng Hospital of the Navy Medical University between 2019 and 2021.Data on general health status and disease characteristics were collected from all patients.With their consent,participants underwent audiological evaluatons and completed questionnaires to analyze the characteristics of their tinnitus and the factors influencing its severity.Results:The correlation analysis showed a positive relationship between tinnitus frequency,tinnitus loudness,SAS scores,and PSQI scores with THI scores(P<0.05)among nine examined variables(gender,handedness,employment status,age,BMI,tinnitus frequency,tinnitus loudness,SAS scores,and PSQI scores).The variables that were extracted from the multiple regression were;for the constant;β=-51.797,t=-4.484,P<0.001,variable is significant;for the tinnitus loudness;β=0.161,t=2.604,P<0.05,variable is significant;for the tinnitus frequency;β=0.000,t=1.269,P=0.206,variable is not significant;for the SAS scores;β=1.310,t=7.685,P<0.001,variable is significant;for the PSQI scores;β=1.680,t=5.433,P<0.001,variable is significant.Therefore,the most accurate model for predicting severity in tinnitus patients is a linear combination of the constant,tinnitus loudness,SAS scores,and PSQI scores,Y(Tinnitus severity)=β0+β1(Tinnitus loudness)+β2(SAS scores)+β3(PSQI scores).β0,β1,β2,and β3 are-51.797,0.161,1.310 and 1.680,respectively.Conclusion:Tinnitus severity is positively associated with loudness,anxiety levels,and sleep quality.To effectively manage tinnitus in patients,it is essential to promptly identify and address these accompanying factors and related symptoms.

    查看更多>>摘要:Background:Nasal packs are central to nasal surgeries.Primarily,these packs function by controlling bleeding,modulating pain and reducing adhesions post-surgery.However,the major setback of the currently used conventional nasal packs is the unbearable pain the patient undergoes upon removal of these packs.To overcome this shortcoming a variety of biodegradable packs have emerged.This study was aimed at evaluating the safety,efficacy and tolerability of VELNEZ nasal packs.VELNEZ,a patented Datt Mediproducts Pvt.Ltd.nasal pack,is one of its kind biodegradable composite that fragments within a few days of application.Methods:Eighty patients were included in an open label,interventional,single arm clinical study using clinical endpoints to investigate the safety and efficacy of nasal pack VELNEZ.The patients were questioned using a visual analog scale from discharge day to 28th postoperative day(9 follow-up visits)at regular intervals.The standardized questionnaires for hemorrhage control,relief from postoperative pain,moderate obstruction,and pain were used.Results:A total of 76 patients were enrolled in the study and 74 patients completed the study.VELNEZ nasal pack played a significant role in controlling hemorrhage and reducing postoperative pain.The average hemorrhage control time was 7.49±3.90 min with only 34.24%of population complaining of pain on the sixth day of surgery(follow-up 4).Forteen days postsurgery only 10.95%of subject population complained of postoperative pain.This biodegradable composite has an average fragmentation time of 4.7 days in the nasal cavity.In addition,this study did not observe any postoperative adverse events or serious adverse events.Conclusion:VELNEZ,a fragmentable nasal pack,is comfortable,safe,and effective against postsurgery bleeding and pain.

    Isolated traumatic orbital floor fracture:An analysis of the National Trauma Data Bank

    查看更多>>摘要:Objectives:To date,no study provides a comprehensive analysis of traumatic orbital floor fractures across the United States.We aimed to characterize patient demographics,injury-related variables,and operative management in this population.Method:The National Trauma Data Bank was queried for open or closed orbital floor fractures from 2008 to 2016.Clinical data were extracted.Results:Overall,148,592 orbital floor fractures were identified,with 142,577(95.9%)closed-and 6158(4.1%)open-type fractures.A total of 106,243(71.5%)patients were male and the median patient age was 41 years.The majority of patients(79.2%)had abbreviated injury scale scores of ≤2,indicating minor/moderate injury.Fracture mechanism of injury(MOI)differed by gender,with the most frequent being unarmed fights in men(34.3%)and falls in women(14.0%).There were 29,600 patients(19.9%)with isolated orbital floor(I-OF)fractures.The MOI most strongly associated with operative intervention of with I-OF fractures were penetrating injuries caused by a firearm(odds ratio[OR]:2.91;95%confidence interval[CI]:1.62-5.20)and cuttings/piercings(OR:2.17;95%CI:1.29-3.65).Conclusion:This large epidemiological study reveals that orbital floor fractures tend to present with minor or moderate injuries and are more likely to require operative intervention in setting of firearm or cut/pierce injuries.

    A randomized controlled trial on triamcinolone versus saline impregnated merocel post endoscopic sinus surgery:Our experience in a tertiary care centre

    查看更多>>摘要:Purpose:To assess the potential benefit of impregnating Merocel(a non-absorbable nasal dressing)with a topical steroid solution,for use as a direct and slow local delivery system of steroids after sinus surgery to improve postoperative wound healing.Methods:In this randomized controlled trial,40 patients with bilateral chronic rhinosinusitis with nasal polyposis were subjected to functional endoscopic sinus surgery.Following the completion of the surgery,Merocel packs were inserted in the bilateral nasal cavities and infiltrated with 4mL triamcinolone(40mg/mL)in one nasal cavity(treatment group)and 4mL normal saline in the other(control group).Nasal packs were removed on the third postoperative day and postoperative healing assessment was done on postoperative Weeks 1,2,4,and 12.The findings were noted as per Lund Kennedy(LKES)and perioperative sinus endoscopy(POSE)scores and compared on both sides.Results:Significant(P<0.05)improvement was noted in Lund Kennedy score for crusting and polyp at Week 12,for edema at Week 1,and nasal discharge at Weeks 1 and 12,but there was no significant improvement in scarring at any week.Overall,the difference between the treatment and control arms was statistically significant at all postoperative visits except at Week 4.Also,there was a significant improvement in POSE scores at Weeks 1,2,and 12 but not at Week 4.Conclusion:This study positively concludes that the nasal cavity packed with drug-soaked packs had less scarring and edema in the postoperative period and the overall wound healing was much better as compared to saline-soaked packs.

    查看更多>>摘要:Objectives:Chronic rhinosinusitis is one of the common diseases that cause morbidity and affects a person's quality of life.We tried to provide a more appropriate and effective approach to selecting patients for endoscopic sinus surgery.Methods:The study population is chronic rhinosinusitis children referred to the ear,nose,and throat clinic of two general hospitals in Tehran,Iran,who have previously undergone sufficient drug treatment and have not recovered.The Lund-Mackay score is calculated by examining the computed tomography(CT)scan.The Sino-nasal Outcome Test-22(SNOT-22)questionnaire was provided to the patients before the operation,after the operation,and 3 and 6 months later in the clinic.Results:Before the operation,the most SNOT-22 score people were in the range of 40-59 points.The SNOT-22 score before the operation is significantly different from 3 and 6 months after the operation.The highest frequency of Lund-Mackay CT(LMCT)scan score was in the range of 18-23 points.The LMCT scan score did not show any significant relationship with the SNOT-22 score before surgery,3 months,and 6 months after surgery.Sensitivity to aspirin had a significant relationship with SNOT-22 scores and the history of asthma and nasal polyps had a significant relationship with the preoperative LMCT scan score.Conclusions:The LMCT scan scoring system cannot be a good measure of chronic rhinosinusitis severity or the prognosis of patients after surgery.The SNOT-22 questionnaire can be used as a predictive tool to help the doctor and the patient in deciding to operate and the possibility of obtaining a relative recovery.

    查看更多>>摘要:Objective:To assess the effect of selective serotonin reuptake inhibitors(SSRIs)and tricyclic antidepressants(TCAs)in reducing vertigo,tinnitus,and hearing loss among patients with Meniere's disease(MD).Data Sources:The following databases were utilized in this scoping review:Ovid Medline,PubMed-NCBI,CINAHL,Cochrane Library,Web of Science,and Clinicaltrials.gov.Method:Studies were identified through the following search phrases:"serotonin specific reuptake inhibitors"OR"tricyclic antidepressants"AND"Meniere's disease."References from included manuscripts were examined for possible inclusion of additional studies.Results:The literature search yielded 23 results,which were screened by three independent reviewers.Seventeen studies and three duplicates were excluded.An examination of references from the included studies yielded two additional publications.A total of four published studies assessing SSRIs and TCAs among 147 patients with MD were ultimately included.Four studies described significant reductions in vertigo attack frequency among patients treated with either SSRIs or TCAs compared to their pretreatment baseline.Three studies assessed the drugs'effects on hearing,of which none found a significant difference among patients treated with SSRIs or TCAs.One study found a significant decrease in patient-reported tinnitus following treatment with TCAs or SSRIs compared to their pretreatment baseline.Conclusions:Data exploring SSRIs and TCAs among patients with MD suggests that these medications may reduce the frequency of tinnitus and vertigo,although there was significant heterogeneity in outcome reporting.There remains a need for larger-scale prospective studies that emphasize objective data to evaluate their effective-ness in reducing common MD symptoms.

    Papillary thyroid carcinoma:A bibliometric analysis of the top 100 cited articles

    查看更多>>摘要:Objectives:To describe the current trends and key themes that are shaping the literature surrounding papillary thyroid carcinoma.Methods:The Thomson Reuters Web of Science citation database was used to identify manuscripts relevant to papillary thyroid carcinoma.These were then subdivided into title,author,journal,publication date,theme,and manuscript types.Themes were then identified.Results:A total of 1506 manuscripts were returned.The mean citation number was 163(range 885-74),with the highest citation rate of 73.5.The highest impact factor was JAMA(impact factor of 56.27).85%were original research,10%review articles,and 5%were guidelines.The highest number of manuscripts were published in South Korea(21%).Conclusions:This bibliometric analysis describes the most influential papers published in the topic of papillary thyroid carcinoma.Historic key themes have been endocrinology and metabolism,followed by surgical management and oncology(subdivided into surgery,pathology,radiology and public health/epidemiology).South Korea leads the world with the largest number of publications in this field;with an incidence rate of papillary thyroid carcinoma that is significantly higher than most parts of the world.Despite concerns with overdiagnosis,a collaborative radiological team skilled in ultrasonography of the neck appears to be of great value to the workup of thyroid cancers.

    查看更多>>摘要:Objective:This comprehensive review aims to explain the disease pathophysiology,clinical presentation,and management options.Methods:A review was carried out in the following databases:Medline,Scopus,Web of Science,and Cochrane.The following terms were used alone and combined:Retrograde,Cricopharyngeus muscle,Dysfunction,Abelchia,and inability to burp.Results:A total of 68 articles were identified,and only 11 were found to be relevant and included in writing this review.Retrograde cricopharyngeal dysfunction(R-CPD)is a relatively new disease entity that has recently been described in clinical literature.It is caused by the inability of the cricopharyngeus muscle to relax.Unlike cricopharyngeal dysfunction(CPD),which is well-known and characterized by dysphagia,R-CPD is characterized by the inability to belch in almost all patients,which is considered diagnostic for the condition.Conclusions:High-resolution manometry(HRM)is the definitive diagnostic modality.Most patients reported in the literature responded well to treatment with botulinum toxin injection.
