花粉与水果交叉反应的研究进展与临床意义

花粉作为常见的吸入性致敏物质,与水果中常见的变应原蛋白存在交叉反应性。水果变应原蛋白高级结构中的分子表位具有免疫原性,这些分子表位的结构域与特异性免疫球蛋白E(immunoglobulin E,IgE)相互作用,引起严重程度不一的过敏反应。花粉与水果交叉过敏反应引起复杂的临床表现,其诊断、治疗和管理问题亟待解决。通过花粉与水果致敏蛋白表位鉴定的方法来诊断引起交叉反应的过敏原,以预防和治疗过敏性疾病。现综述水果泛过敏原与花粉过敏原的各表型特征、植物分布特点、区域性以及引起交叉反应相关临床症状、组分诊断,重点关注蔷薇科水果泛变应原与多种植物花粉致敏蛋白的相关特性,以及中国地区蒿属花粉与水果交叉过敏的特点。
Progress and clinical significance of cross-reaction between pollen and fruit
Pollen,as a common inhalant sensitizer,cross-reacts with common allergen proteins in fruits.Molecular epitopes in higher order structures of fruit allergen proteins are immunogenic and these molecular epitope domains interact with specific IgE causing aller-gic reactions of varying severity.Pollen and fruit cross-allergic reactions cause complex clinical manifestations,and their diagnosis,treatment and management problems need to be solved urgently.Allergens causing cross-reactivity are diagnosed by epitope identifica-tion of pollen and fruit allergens to prevent and treat allergic diseases.This article reviews the phenotypic characteristics,plant distribu-tion characteristics,regionality,clinical symptoms and component diagnosis of fruit panallergens and pollen allergens,focusing on the related characteristics of rosaceae fruit panallergens and a variety of plant pollen sensitizing proteins,as well as the characteristics of Artemisia pollen and fruit cross-allergy in China.The identification of allergens is the direction of accurate diagnosis and treatment of allergic diseases and has guiding significance for clinical work.
