Effect of catgut embedding at eight acupoints around the umbilicus on pregnancy outcome,glucose and lipid metabolism and TCM syndrome score of obese patients with PCOS infertility undergoing IVF-ET
Objective To explore the effects of catgut embedding at eight acupoints around the navel on pregnancy outcomes,glucose and lipid metabolism,and traditional Chinese medicine syndrome scores in obese polycystic ovary syndrome(PCOS)infertility patients undergoing in vitro fertilization embryo transfer(IVF-ET).Methods Totally 103 cases of obese PCOS infertility patients,who under-went IVF-ET in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from October 2020 to March 2022,were randomly assigned into control group(n=51,conventional IVF-ET treatment)and treatment group(n=52,conventional IVF-ET treat-ment combined with catgut embedding at eight acupoints around the umbilicus)by random number table method.A comparison was made between the two groups of the TCM syndrome scores,psychological status[Hamilton anxiety rating scale(HAM-A),Hamilton rat-ing scale for depression(HAM-D)],glucose and lipid metabolism indicators[fasting blood glucose(FBG),fasting insulin(FINS),homeo-stasis model assessment insulin resistance(HOMA-IR)],blood lipid indices[triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),total cholesterol(TC)],body mass index(BMI),waist to hip ratio(WHR),inflam-matory factors[interleukin-6(IL-6),interleukin-10(IL-10),tumor necrosis factor-α(TNF-α)]and pregnancy outcomes.Results After treatment,the TCM syndrome scores,HAMA scores,HAMD scores,FBG,FINS and HOMA-IR levels of the two groups were decreased compared with those before treatment.After treatment,the levels of the above indexes in the treatment group were(6.02±1.68),(7.31±1.29),(6.86±1.39),(4.04±0.64)mmol/L,(10.04±0.64)mIU/L,and(1.61±0.04),respectively,which were lower than those in the control group(10.19±2.21),(10.42±2.36),(9.62±2.64),(5.32±1.02)mmol/L,(14.45±1.19)mIU/L,and(2.11±0.06)(P<0.05).After treatment in the treatment group,the levels of IL-6,TNF-α,TG,LDL-C,TC,BIM,WHR were(6.67±1.38)ng/L,(22.18±2.23)ng/L,(1.21±0.05)mmol/L,(2.65±0.36)mmol/L,(3.12±0.45)mmol/L,(24.89±1.19)kg/m2,and(0.87±0.06),which were lower than those in the control group(P<0.05).The levels of IL-10 and HDL-C were(167.46±31.25)ng/L,and(2.01±0.19)mmol/L,respectively,which were higher than those in the control group[(125.03±24.11)ng/L,(1.32±0.21)mmol/L](P<0.05).There was no difference in the clinical pregnancy rate between the treatment group and the control group(53.84%vs.41.17%)(χ2=1.66,P=0.198);the abortion rate of the treatment group was 3.57%,lower than that of the control group,which was 23.81%(χ2=4.57,P=0.032).No adverse reactions occurred in both groups.Conclusion The treatment of catgut embedding at eight acupoints around the umbilicus is helpful to reduce the TCM syn-drome score of obese patients with PCOS infertility,and it can improve patients'negative emotions and inflammatory reactions,regulate glucose and lipid metabolism,and reduce miscarriage rate,which is safe and reliable.
