Effect of Transcutaneous Electrical Nerve Stimulation on Upper Limb Function in Stroke Patients:A Meta-Analysis

目的:系统评价经皮神经电刺激(TENS)对脑卒中患者上肢功能的影响.方法:全面检索8个数据库,包括中国知网、中国维普、万方、PubMed、Web of Science、EMBASE、Cochrane Library、CINAHL,严格按照纳入排除标准进行文献的筛选.采用Cochrane系统对纳入文献进行质量评价,采用Revman 5.4.1软件进行Meta分析.结果:共纳入了 11项随机对照研究,包括559名患者,Meta分析结果显示,TENS可以提高脑卒中上肢功能障碍患者的Fugl-Meyer上肢评估(FMA-UE)量表评分(MD=5.77,95%CI:3.12,8.42)、降低改良 Ashworth 肌张力评定(MAS)量表评分(MD=-0.38,95%CI:-0.65,-0.11)、提高改良 Barthel 指数(MBI)评分(MD=10.68,95%CI:8.28,13.09).亚组分析结果显示,是否偏瘫不是产生异质性的来源.结论:TENS可以提高脑卒中上肢功能障碍患者的FMA-UE量表和MBI评分,降低MAS量表评分,对脑卒中患者的上肢功能恢复有积极影响.
Objective:To systematically evaluate the effect of transcutaneous electrical nerve stimulation(TENS)on upper limb function in stroke patients.Methods:A comprehensive search was conducted across eight databases,including CNKI,VIP,Wanfang,PubMed,Web of Science,EMBASE,Cochrane Library,and CINAHL,strictly adhering to the inclusion and exclusion criteria for literature screening.The quality of the included studies was assessed using the Cochrane system,and a meta-analysis was performed using Revman 5.4.1 software.Results:A total of 11 randomized controlled studies,including 559 patients were included.The meta-analysis showed that TENS could improve the Fugl-Meyer Upper Extremity Assessment(FMA-UE)scores(MD=5.77,95%CI:3.12,8.42),reduce the Modified Ashworth Scale(MAS)scores(MD=-0.38,95%CI:-0.65,-0.11),and increase the Modified Barthel Index(MBI)scores(MD=10.68,95%CI:8.28,13.09)in stroke patients with upper limb dysfunction.Subgroup analysis results indicated that hemiplegia was not a source of heterogeneity.Conclusion:TENS can improve the FMA-UE and MBI scores and reduce the MAS scores in stroke patients with upper limb dysfunction,having a positive impact on the recovery of upper limb function in these patients.
