摘要
目的 分析2018年4月至2023年3月北京清华长庚医院成年患者血清25-羟基维生素D[25(OH)D]水平的分布特征和变化趋势.方法 共纳入9470例在2018年4月至2023年3月于北京清华长庚医院进行血清25(OH)D检测的患者,冋顾性分析患者25(OH)D水平在不同性别、年龄、检测季节和年份之间的差异.结果 全体受试者的25(OH)D水平的中位数(四分位数)为16.52(11.58,23.26)ng/mL,其中有6019(63.6%)例患者为维生素D缺乏(<20ng/mL),2398(25.3%)例患者为维生素D不足(20~30ng/mL),仅有1 053(11.1%)例患者为维生素D充足状态(>30ng/mL).患者的25(OH)D水平在不同年龄间和检测的不同季节间差异明显,主要表现为18~39岁年龄组人群较低,春季和秋季水平较低.从2018年到2023年5年间逐年分析显示患者的25(OH)D水平没有明显随年份的变化趋势.结论 目前医院患者的维生素D水平偏低,18~39岁年龄组人群和春季、冬季检测的患者中25(OH)D缺乏的比例较高,提示临床医护应关注就诊患者的维生素D缺乏状态,必要时为患者补充维生素D或建议增加户外运动.
Abstract
Objective To analyze the distribution characteristics and change trend of serum 25 hydroxyvitamin D[25(OH)D]concentration of adult patients from Beijing Tsinghua Changgung Hospital from April,2018 to March,2023.Methods A total of 9 470 patients who have undergone 25(OH)D testing at Beijng Tsinghua Changgung Hospital from April,2018 to March,2023 were included in this study,and 25(OH)D concentration of all patients were retrospectively analyzed by using adjusting factor of sex,age,testing season and year of testing.Results The median(interquartile range)of 25(OH)D concentration of the patients included was 16.52(11.58,23.26)ng/mL,of which 6 019(63.6%)showed vitamin D-deficiency(<50nmol/L),2398(25.3%)showed vitamin D-insufficiency(50-75nmol/L),and only 1 053(11.1%)showed vitamin D-sufficiency(>75nmol/L).25(OH)D concentration were significant different between patients'ages and seasons of testing,showing lower 25(OH)D concentration for 18-39-year group,and lower 25(OH)D concentration testing in spring and fall.There was no significant year-to-year trend in 25(OH)D concentration analyzed year by year over a 5-year period from 2018 to 2023.Conclusion At present,the overall vitamin D concentration level of patients is relatively low,and the proportion of patients with 25(OH)D deficiency in 18-39 year group and patients tested in spring and winter season is high,which suggests that more attention should paid to patients'vitamin D deficiency,and to provide the following advice to patients,such as replenishing vitamin D in time or increasing outdoor exercise.
