血清SDC4、MSTN联合检测对心房颤动射频消融术后复发的预测价值

摘要
目的 探讨血清多配体蛋白聚糖4(SDC4)、肌肉生长抑制素(MSTN)水平对心房颤动(AF)患者射频消融术后复发的预测价值.方法 选取2016年6月至2019年9月在本院接受治疗的AF患者98例为AF组,选择同期体检的健康者96例作为对照组.采用酶联免疫吸附测定法(ELISA)检测血清SDC4、MSTN水平;对AF患者术后随访36个月,统计术后复发情况,比较复发组与未复发组的一般临床资料与血清SDC4、MSTN水平;采用Logistic回归多因素分析AF患者术后复发的影响因素;受试者工作特征(ROC)曲线分析血清SDC4、MSTN水平对AF患者射频消融术后复发的预测价值.结果 AF组的血清SDC4、MSTN水平均显著高于对照组(P＜0.05);98例AF患者中术后复发40例,术后复发率为40.82％;复发组的左心房直经、血清SDC4、MSTN水平高于未复发组(P＜0.05);血清SDC4、MSTN水平是影响AF患者术后复发的独立危险因素;血清SDC4、MSTN预测AF患者射频消融术后复发的线下面积(AUC)分别为0.876、0.803,截断值分别为5.80ng/L、27.33μg/L,灵敏度分别为87.5％、87.5％,特异性分别为74.1％、65.5％,且SDC4、MSTN联合检测预测AF患者射频消融术后复发的AUC为0.923,灵敏度为90.0％,特异性为77.6％.结论 血清SDC4、MSTN水平是AF患者射频消融术后复发的独立危险因素,二者可以作为AF患者术后复发的预测指标,且二者联合检测的预测价值更高.
Abstract
Objective To investigate the predictive value of serum polyligand proteoglycan 4(SDC4)and myostatin(MSTN)levels in patients with atrial fibrillation(AF)after radiofrequency ablation.Methods A total of 98 patients with AF who were treated in our hospital from June,2016 to September,2019 were regarded as the AF group,while 96 healthy people who were examined during the same period were regarded as the control group.Levels of SDC4 and MSTN in serum were detected by enzyme-linked immunosorbent assay(ELISA)in all subjects;we followed up AF patients for 36 months after operation.The postoperative recurrence was counted,and the general clinical data and serum SDC4 and MSTN levels were compared between the recurrence group and the non-recurrence group;Logistic regression was used to analyze the influencing factors of postoperative recurrence in AF patients.The predictive value of serum SDC4 and MSTN levels in AF patients after radiofrequency ablation was analyzed by receiver operating characteristic(ROC)curve.Results The serum SDC4 and MSTN levels in AF group were significantly higher than those in the control group(P＜0.05);Among 98 patients with AF,40 had postoperative recurrence,and the postoperative recurrence rate was 40.82％;The left atrial straight channel and the levels of serum SDC4 and MSTN in the recurrence group were higher than those in the non-recurrence group(P＜0.05);Serum SDC4 and MSTN levels were independent risk factors for postoperative recurrence of AF patients;AUCs of serum SDC4 and MSTN for recurrence prediction in AF patients after radiofrequency ablation were 0.876 and 0.803,respectively,and the cut-off value were 5.80ng/L and 27.33μg/L,respectively,with sensitivity of 87.5％and 87.5％,and specificity of 74.1 and 65.5％,respectively.AUC of SDC4 combined with MSTN in predicting recurrence of AF patients after radiofrequency ablation was 0.923,wiht the sensitivity of 90.0％,and the specificity of 77.6％.Conclusion Serum SDC4 and MSTN levels are independent risk factors for recurrence in AF patients after radiofrequency ablation,and these markers can be used as predictive indicators of recurrence in patients with AF.Their combined predictive value is even higher.
