The Predictive Value of Lipids in Patients with Type 2 Diabetes Mellitus with Poor Glycemic Control
Objective The objective of this study is to examine variations in blood lipids among individuals diagnosed with type 2 diabetes mellitus(T2DM)who exhibit varying degrees of blood glucose control.Furthermore, this research aims to assess the potential predictive capacity of blood lipids in T2DM patients with inadequate blood glucose control. Methods The clinical data of 89 patients diagnosed with type 2 diabetes mellitus(T2DM)at China National Nuclear Corporation 416 Hospital between January,2021 and December,2022 were gathered and subsequently categorized into two groups based on their HBA1c levels. These datasets were then subjected to a retrospective analysis. Results Concentrations of cholesterol(CHO),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),apolipoprotein B(ApoB),ApoB/ApoA1,and small but dense low-density lipoprotein(sdLDL-C)were found to be significantly elevated in the poorly controlled blood glucose group compared to the well-controlled blood glucose group(P<0.05).After adjusting for covariates including age, sex, and BMI,the results showed significant positive correlations between HBA1c and several biomarkers, including CHO(r=0.318,P=0.003),NEFA(r=0.296,P=0.003),LDL-C(r=0.404,P=0.001),ApoB(r=0.349,P=0.001),ApoB/ApoA1(r=0.341,P=0.001),and sdLDL-C(r=0.315,P=0.003).Furthermore,ApoB,LDL-C,and sdLDL-C were found to be predictive for poor glycemic control in patients with type 2 diabetes mellitus(ApoB:sensitivity of 65.1%and specificity of 80.4%;LDL-C:sensitivity of 81.4%and specificity of 67.7%;sdLDL-C:sensitivity of 69.8%and specificity of 71.7%).Conclusion Levels of CHO,NEFA,LDL-C,ApoB,sdLDL-C,and ApoB/ApoA1 exhibit a significant correlation with glycated hemoglobin levels in patients with type 2 diabetes mellitus(T2DM).Furthermore,ApoB,LDL-C,and sdLDL-C demonstrate a strong predictive capacity for T2DM patients who have inadequate glycemic control.
