The Diagnostic Value of Serum CXCL16 and ESM-1 Levels for Cognitive Impairment in Patients with Cerebrovascular Disease
Objective To investigate the diagnostic value of serum CXC chemokine ligand 16(CXCL16)and endothelial cell specific molecule 1(ESM-1)levels in cognitive impairment in patients with cerebral small vessel disease(CSVD).Methods A total of 131 CSVD patients admitted to our hospital from October,2019 to October,2022 were selected for the study.According to the scores of the Montreal Cognitive Assessment Scale(MoCA)at the time of admission,these CSVD patients were grouped into a cognitively normal group(60 cases)and a cognitively impairment group(71 cases).Spearman method was applied to analyze the correlation between serum CXCL16,ESM-1 expression levels and MoCA score,Logistic regression was applied to identify the relevant factors affecting cognitive impairment in CSVD patients.Receiver operating characteristic(ROC)curve was drawn to evaluate the diagnostic value of serum CXCL16 and ESM-1 for cognitive impairment in CSVD patients.Results The serum CXCL16 expression level in patients with cognitive impairment was higher than that in the cognitively normal group,while the serum ESM-1 expression level was lower than that in the cognitively normal group(P<0.05);The expression level of serum CXCL16 was negatively correlated with MoCA score(r=-0.767,P<0.05),while the expression level of serum ESM-1 was positively correlated with MoCA score(r=0.723,P<0.05);The proportions of patients with age ≥ 60 years old,history of hypertension,and cerebral infarction in the cognitive impairment group were higher than those in the cognitive normal group(P<0.05);Age,history of hypertension,cerebral infarction,and CXCL16 were risk factors for cognitive impairment in CSVD patients,while ESM-1 was a protective factor(P<0.05);AUCs of cognitive impairment in CSVD patients diagnosed with serum CXCL16,ESM-1,and their combination were 0.748,0.671,and 0.851,respectively,while the combined detection of these two markers was superior to the individual detection of serum CXCL16 or ESM-1(Zcombination-CxCL16=3.284,Zcombination-ESM-1=4.111,P=0.001,<0.001),suggesting a high diagnostic value for cognitive impairment in CSVD.Conclusion Expression levels of serum CXCL16 and ESM-1 are closely related to the occurrence of cognitive impairment in CSVD,and their combination has a high diagnostic value for cognitive impairment in CSVD.
