摘要

目的 了解本地区男性患者人乳头瘤病毒(HPV)感染情况,为其预防与治疗提供数据支持.方法 回顾性分析2020~2022年3年中的临床男性患者HPV感染情况,利用多通道实时荧光定量聚合酶链反应(PCR)进行HPV DNA分型检测,检测型别包括15种HPV高危型(HPV 16、18、31、33、35、39、45、51、52、56、58、59、66、68、82)及2种HPV低危型(HPV 6&11),统计分析不同年龄、不同疾病状态下男性患者HPV感染情况,以及不同HPV亚型单独感染及混合感染情况.结果 2020~2022年3年间共收集7024例男性患者的分泌物标本开展HPV DNA分型检测,其中HPV阳性2793例,检出率为39.76%;不同疾病状态中以皮肤感染和病毒性疣为主;不同年龄段中,以21~30岁感染占比最高为1143例(16.27%),其次为31~40岁1109例(15.79%),与其他年龄段相比,差异具有统计学意义(x2=27.342,P<0.001);在不同HPV感染亚型中,单一亚型感染病例最多为1744例(24.83%)(x2=20.431,P<0.001),二重感染619例(8.81%),三重感染249例(3.54%),四重及以上感染181例(2.58%);在HPV亚型单独感染中,以HPV 6&11亚型(低危型)最高(445例,6.34%),其次为16亚型(229例,3.26%)、52亚型(211例,3.00%);在HPV亚型多重感染中,以高低危混合亚型感染多见,二重感染以HPV 6&11/16最高(135例,1.92%),三重感染以HPV 6&11/16/52最高(29例,0.41%),四重感染以HPV 6&11/51/52/58最高(10例,0.14%).结论 2020~2022年3年间,男性HPV感染高发于21~40岁,主要感染的HPV亚型以6&11、16、52、58、51为主,以单一感染为主,混合感染中以低危和高危混合感染为主.这为本地区男性HPV感染的预防与治疗提供了可靠的数据支持.

Abstract

Objective To analyze the human papillomavirus (HPV)infection status in male patient population in Beijing area,and to provide data-driven support for its prevention and treatment strategies.Methods A retrospective analysis was conducted on the HPV infection status of clinical male patients from 2020 to 2022. Multichannel real-time fluorescence quantitative polymerase chain reaction (PCR)was applied for HPV DNA subtype detection,which included 15 high-risk HPV subtypes (HPV16,18,31,33,35,39,45,51, 52,56,58,59,66,68,82)and 2 low-risk HPV subtypes (HPV 6&11).The HPV infection events of male patients at different age groups and disease types were statistically analyzed.We also analyzed the individual and mixed infections among different HPV subtypes.Results From 2020 to 2022,a total of 7024 male patients'secretion samples were collected for HPV DNA subtype detection.The positive rate of HPV infection was 39.76% (2793 cases).Among different disease types,skin infection and viral warts were the main diseases.The 21-30 age group was the highest proportion of HPV infection with 1143 cases (16.27%),followed by 31-40 aged group with 1109 cases (15.79%).Compared with other age groups,there was a statistically significant difference (x2=27.342,P<0.001 ).Among different HPV subtypes infection,the single subtype infection was the highest with 1744 cases (24.83%)(x2=20.431,P<0.001),followed by double subtype infection (619 cases,8.81%)and triple infection (249 cases,3.54%).Among single HPV subtypes infection,HPV 6&11 subtype (low risk type)was the highest (445 cases,6.34%),followed by 16 subtype (229 cases,3.26%)and 52 subtype (211 cases, 3.00%);Among the multiple infections of HPV subtypes,HPV 6&11/16 were the highest in secondary infections (135 cases,1.92%),while HPV 6&11/16/52 were the highest in triple infections (29 cases,0.41%), and HPV6&11/51/52/58 were the highest in quadruple infections (10 cases,0.14%).Conclusion During the period of 2020 to 2022,the high incidence of HPV infection in male population was between the ages of 21 and 40.The main HPV subtypes infected were 6&11,16,52,58,and 51. Single subtype was the main type infection.This study provides a reliable data-driven support for the prevention and treatment of male HPV infection in this region.
