摘要

目的 分析糖尿病合并甲状腺功能亢进(甲亢)患者血清微小RNA-122(miR-122)和微小RNA-195(miR-195)的表达水平,并探讨其临床诊断价值.方法 选取我院2020年3月至2023年3月98例糖尿病合并甲亢患者为糖尿病合并甲亢组,选取同期收治的98例甲状腺功能正常的糖尿病患者为糖尿病组,另选本院同期98例体检健康志愿者为健康对照组.采用实时荧光定量PCR(qRT-PCR)法检测血清miR-122和miR-195表达水平.Pearson相关性分析糖尿病合并甲亢患者血清miR-122、miR-195水平与游离三碘甲状腺原氨酸(FT3)、游离四碘甲状腺原氨酸(FT4)、促甲状腺激素(TSH)、空腹血糖(FBG)、空腹胰岛素(FINS)及胰岛素抵抗指数(HOMA-IR)的相关性;血清miR-122、miR-195水平对糖尿病合并甲亢的临床诊断价值分析采用ROC曲线进行判断.结果 健康对照组、糖尿病组、糖尿病合并甲亢组患者血清miR-122、miR-195表达水平逐渐增加(P<0.05).多因素Logistic回归分析结果显示,糖化血红蛋白、FT3、FT4、TSH、HOMA-IR及miR-122、miR-195表达是糖尿病合并甲亢的影响因素(P<0.05).Pearson分析显示,糖尿病合并甲亢患者血清miR-122、miR-195水平与FT3、FT4、FBG、FINS及HOMA-IR呈正相关(P<0.05),与TSH呈负相关(P<0.05).ROC曲线分析显示,miR-122和miR-195联合诊断糖尿病合并甲亢的AUC显著大于miR-122单独诊断的AUC(Z=2.245,P=0.025)及miR-195单独诊断的AUC(Z=2.182,P=0.029).结论 糖尿病合并甲亢患者血清miR-122、miR-195表达水平明显升高,二者联合检测对糖尿病合并甲亢具有较高的诊断价值.

Abstract

Objective To analyze the expression levels of serum microRNA-122 (miR-122)and microRNA-195 (miR-195)in patients with diabetes and hyperthyroidism,and to evaluate their clinical diagnostic values.Methods From March,2020 to March,2023,98 patients with diabetes and hyperthyroidism in our hospital were regarded as the diabetes and hyperthyroidism group,while 98 diabetic patients with normal thyroid function admitted during the same period were selected as the diabetic group,and 98 healthy volunteers for physical examination in our hospital in the same period were included as the healthy control group.Real-time fluorescence quantitative PCR (qRT-PCR)was applied to detect expression levels of miR-122 and miR-195 in serum.Pearson method was applied to analyze the correlation between serum miR-122,miR-195 levels and free triiodothyronine (FT3 ),free tetraiodothyronine (FT4 ),thyroid hormone (TSH),fasting blood glucose (FBG),fasting insulin (FINS)and insulin resistance index (HOMA-IR)in patients with diabetes and hyperthyroidism;The clinical diagnostic value of serum miR-122 and miR-195 levels in diabetes with hyperthyroidism was analyzed by ROC curve.Results Expression levels of miR-122 and miR-195 in serum of patients in healthy control group,diabetes group and diabetes with hyperthyroidism group increased gradually (P<0.05).Multivariate logistic regression analysis showed that the expression of glycosylated hemoglobin,FT3,FT4,TSH,HOMA-IR,miR-122,and miR-195 were the influencing factors of diabetes with hyperthyroidism (P<0.05 ).Pearson analysis showed that serum miR-122 and miR-195 levels in patients with diabetes combined with hyperthyroidism were positively correlated with FT3,FT4,FBG,FINS and HOMA-IR (P<0.05),while negatively correlated with TSH (P<0.05).ROC curve analysis showed that the AUC of miR-122 combined with miR-195 in the diagnosis of diabetes with hyperthyroidism was significantly higher than that of miR-122 (Z=2.245,P=0.025)or miR-195 alone (Z=2.182,P=0.029).Conclusion Expression levels of miR-122 and miR-195 in serum of patients with diabetes and hyperthyroidism are significantly increased.The combined detection of miR-122 and miR-195 has a high diagnostic value for diabetes and hyperthyroidism.
