老年患者下呼吸道感染鲍曼不动杆菌耐药性分析

摘要
目的 探讨由鲍曼不动杆菌引起下呼吸道感染的老年患者对常用抗菌药物的耐药性.方法 对石家庄市人民医院住院病房2021年1月至2023年12月送检的老年患者下呼吸道样本检出的578株鲍曼不动杆菌临床分布及耐药性进行分析.结果 3年期间住院病房老年患者送检样本共分离出654株鲍曼不动杆菌,分离率为8.91%(654/7341),占第四位,其中下呼吸道来源578株,构成比为88.38%.老年患者检出鲍曼不动杆菌以重症监护室检出率最高(27.22%).578株鲍曼不动杆菌对亚胺培南的耐药率从2021年的26.76%,上升至2023年的56.30%;对头孢他啶、头孢曲松和头孢吡肟的耐药率从2021年的28.17%、17.95%和25.53%,上升至2023年的54.24%、50.50%和46.81%;对哌拉西林/他唑巴坦的耐药率从2021年的33.33%,上升至2023年的59.57%;对阿米卡星和妥布霉素的耐药率从2021年的8.45%和28.13%分别上升至2023年的22.77%和50.37%;对左旋氧氟沙星的耐药率从2021年的21.13%上升至2023年的33.90%.结论 老年患者下呼吸道来源鲍曼不动杆菌构成比较高,对临床常用抗菌药物普遍高度耐药,临床需加强抗菌药物的合理应用.
Abstract
Objective To investigate the resistance of elderly patients with lower respiratory tract infections caused by Acinetobacter baumannii to commonly used antibiotics.Methods The clinical distribution and resistance rate of 578 strains of Acinetobacter baumannii detected in lower respiratory tract samples from elderly patients in the Inpatient Ward of Shijiazhuang People's Hospital from January,2021 to December,2023 were analyzed.Results During the 3-year period,a total of 654 strains of Acinetobacter baumannii were isolated from samples of elderly patients in the Inpatient Ward,with an isolation rate of 8.91%(654/7341),ranking as the fourth in all strains.Among them,578 strains were from the lower respiratory tract,accounting for 88.38%of the total strains.The detection rate of Acinetobacter baumannii in elderly patients was the highest in the Intensive Care Unit(27.22%).The resistance rate of 578 strains of Acinetobacter baumannii to imipenem increased from 26.76%in 2021 to 56.30%in 2023;The resistance rates to ceftazidime,ceftriaxone,and cefepime increased from 28.17%,17.95%,and 25.53%in 2021 to 54.24%,50.50%,and 46.81%in 2023,respectively;The resistance rate to piperacillin/tazobactam increased from 33.33%in 2021 to 59.57%in 2023;The resistance rates to amikacin and tobramycin increased from 8.45%and 28.13%in 2021 to 22.77%and 50.37%in 2023,respectively;The resistance rate to levofloxacin increased from 21.13%in 2021 to 33.90%in 2023.Conclusion The composition of Acinetobacter baumannii in the lower respiratory tract of elderly patients is relatively high,and they are generally highly resistant to commonly used antibiotics in current clinical practice.To address this,it is necessary to further strengthen the rational use of antibiotics.
