An Analysis of the Value of Color Doppler Ultrasound Combined with Platelet-to-lymphocyte Ratio in the Preoperative Diagnosis of Differentiated Thyroid Carcinoma
Objective To evaluate the value of color Doppler ultrasound combined with platelet to lymphocyte ratio(PLR)in the preoperative diagnosis of differentiated thyroid cancer.Methods A retrospective analysis was conducted on the medical records of 95 patients with differentiated thyroid cancer admitted to the Second People's Hospital of Chizhou City from February,2020 to August,2023.In addition,95 patients diagnosed as benign thyroid nodules with color Doppler ultrasound results were selected as the control group.We collected and processed preoperative color Doppler ultrasound and PLR examination data of all subjects,then compared the ultrasound imaging characteristics and PLR levels of two groups of thyroid nodule patients,and evaluated the diagnostic efficacy of color Doppler ultrasound and PLR for differentiated thyroid cancer.Results The proportion of differentiated thyroid carcinoma patients with solitary nodules,nodules with a maximum diameter ≤1cm,and the presence of calcifications was higher than that of patients with benign thyroid nodules(P<0.05).Kappa values for the consistency between the preoperative diagnoses of differentiated thyroid carcinoma using color Doppler ultrasound,PLR,and their combination,and the postoperative pathological diagnoses were 0.679,0.640,and 0.723,respectively.PLR levels in patients with differentiated thyroid carcinoma were found to be higher than those in patients with benign thyroid nodules(P<0.05).The sensitivities of color Doppler ultrasound alone,PLR alone,and their combination in diagnosing differentiated thyroid carcinoma were 84.21%,82.10%,and 86.32%,respectively.The specificities were 71.58%,73.68%,and 74.74%,respectively.The area under the curve(AUC)values were 0.767,0.810,and 0.881,respectively.The Delong test indicated that the diagnostic performance of the combined method was significantly superior to that of the single-marker method(P<0.05).Conclusion The combination of color Doppler ultrasound and PLR shows a good performance in the preoperative diagnosis of differentiated thyroid carcinoma.
