A Study on the Diagnostic Value of Serum Sex Hormone Basic Value for Central Precocious Puberty in Girls with Early Breast Development
Objective To compare and analyze basic levels of serum sex hormones between girls with premature thelarche and central precocious puberty(CPP),and to explore the diagnostic values and cut-off values of serum sex hormone basic values for CPP in girls with early breast development.Methods 92 girls in the breast development group and 53 girls with CPP were selected based on clinical manifestations,physical examinations,laboratory tests,and imaging examinations.Retrospective analysis was conducted on the serum basal sex hormone levels of the children at initial diagnosis.We collected data on serum basal luteinizing hormone(LH),follicle stimulating hormone(FSH),progesterone(P),estradiol(E2),pituitary prolactin(PRL),and total testosterone(T)levels in two groups of pediatric patients.Serum FSH,E2,and PRL information were metric data that conform to normal distribution.SPSS 13.0 statistical software was used to conduct t-test statistical analysis to compare the basic values of various sex hormones in the serum of the two groups of children;Serum LH,P,and T were measurement data that did not conform to normal distribution.Non-parametric t-test Mann Whitney rank sum test was used for statistical analysis to compare basic values of serum sex hormones between the two groups of children.Using the premature thelarche group as the control group and the CPP group as the observation group,we drew the subject working characteristic curve(ROC)and determined the cut off value.Results t-test analysis showed that there was no statistically significant difference in serum basal FSH(2.44±1.19IU/L vs 2.50±1.39IU/L,t=0.29,P=0.77)and PRL(7.2±4.07ng/m vs 7.75±4.10ng/mL,t=0.78,P=0.44)between the premature thelarche group and the CPP group.There was a statistically significant difference in E2(28.26±9.50pg/mL vs 32.31±15.02pg/mL,t=1.99,P=0.049)between the two groups,and the CPP group was significantly higher than the premature thelarche group.Through non-parametric t-test Mann Whitney rank sum test analysis,the serum basal LH(0.69 IU/L,P=0.0001)and T(0.21ng/mL,P=0.0001)in the CPP group were significantly higher than those in the premature thelarche group,and p value(0.59ng/mL,P=0.20)suggested no significant difference.The areas under the ROC curves of LH,E2,and T used for diagnosing CPP in girls with early breast development were 0.72,0.56,and 0.71,respectively.The cut-off values were LH 0.64 IU/L,E2 40.23pg/mL,and T 0.23ng/mL,respectively.Conclusion Serum basal LH,E2,and T levels in the CPP group are significantly higher than those in the premature thelarche group.Serum basal LH,E2,and T levels have certain value in the differential diagnosis of central precocious puberty and partial precocious puberty.LH≥0.64IU/L or E2≥40.23pg/mL or T≥0.23ng/mL,can be used as preliminary screening indicators for central precocious puberty.
