Clinical observation of repositioning maneuvers in the treatment of positional down beat nystagmus with vertigo
Objective To explore the effect of repositioning maneuvers on the treatment of positional down beat nystagmus(pDBN)with vertigo.Methods A total of 33 patients with pDBN accompanied with vertigo in Tongzhou Hospital of Dongzhimen Hospital Affiliated to Beijing University of Chinese Medicine from January 2019 to December 2023 were selected.The clinical effect of repositioning maneuvers on pDBN patients with vertigo was analyzed.Results Among 33 patients,there were 13 males and 20 females,aged from 22 to 79 years,with an average age of(55.8±15.5)years.Repositioning maneuvers was effective in 18 patients.The effective rate of Yacovino maneuver was 33.3%,and that of Yacovino combined with semi-Semont maneuver was 21.2%.A total of four patients converted to typical posterior semicircular canal benign paroxysmal positional vertigo(BPPV)during treatment.The positional nystagmosis features of anterior semicircular canal BPPV were similar to that of contralateral apogeotropic posterior semicircular canal BPPV.Conclusions Yacovino maneuver combined with semi-Semont maneuver can improve the success rate of diagnosis and reduction treatment in case with pDBN.
