Correlation analysis of serum 25-hydroxyvitamin D3 and CRP level with antiviral efficacy and inflammatory activity in patients with chronic hepatitis B
Objective To explore the correlation between serum 25-hydroxyvitamin D3[25(OH)D3]levels,CRP and antiviral efficacy and inflammatory activity in patients with chronic hepatitis B(CHB).Methods A total of 106 CHB patients in the Second Traditional Chinese Medicine Hospital of Kaifeng City from December 2022 and December 2023 were selected,and were divided effective group(n=68)and ineffective group(n=38)according to whether HBV DNA turned negative or not.At the same time,they were divided into G1 stage(n=11),G2 stage(n=36),G3 stage(n=44)and G4 stage(n=15)according to the classification of inflammatory activity.Serum 25(OH)D3,CRP,and liver function markers(ALT,AST,GGT,and T-BIL)were compared among the subgroups.The correlation between indicators and inflammatory activity was analyzed by Pearson correlation,and influencing factors of antiviral efficacy in CHB patients was analyzed by multivariate logistic regression.The predictive value of serum 25(OH)D3 and CRP on antiviral efficacy in patients with CHB was analyzed by ROC curve.Results Among 106 patients with CHB,there were 58 males and 48 females,aged from 24 to 68 years,with an average of(47.6±12.3)years.The serum 25(OH)D3 levels showed a downward trend with increasing inflammation stages,whereas CRP,ALT,AST,GGT,and T-BIL levels increased,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that the inflammatory activity of CHB patients was positively correlated with CRP,ALT,AST,GGT and T-BIL(P<0.05)and negatively correlated with 25(OH)D3(P<0.05).The 25(OH)D3 was higher and CRP levels was lower in the effective group than that of the ineffective group,and the differences were statistically significant(P<0.05).Multivariate logistic regression analysis showed that the lower the level of 25(OH)D3(OR=0.382,95%CI:0.131-0.885,P=0.000)and the higher the level of CRP(OR=6.615,95%CI:2.855-14.208,P=0.000),the worse the antiviral effect of CHB patients.ROC analysis showed that the AUC,sensitivity and specificity of serum 25(OH)D3 and CRP in predicting the antiviral efficacy of CHB patients were 0.812,0.8083 and 0.8545,respectively.Conclusions Serum 25(OH)D3 and CRP in patients with CHB are related to antiviral efficacy and inflammatory activity,and their combination has high predictive value for antiviral efficacy in patients with CHB.
