Analysis of Candida strains distribution and drug resistance changes in an infectious disease hospital in Beijing from 2019 to 2023
Objective To explore the distribution and drug resistance of Candida strains in an infectious disease hospital in Beijing from 2019 to 2023.Methods A total of 723 patients with Candida infection and 753 strains in Beijing You'an Hospital,Capital Medical University from 2019 to 2023 were selected,and their strains distribution and drug resistance characteristics were analyzed.Results Among the 723 patients,there were 276 males and 447 females,aged from 22 to 91,with a median age of 59 years.Candida albicans,Candida tropicalis,Candida glabrata and Candida parapsilosis were the highest detection rates in recent five years.The distribution of strains in patients with liver disease is consistent with the overall distribution,and the proportion of Candida parapsilosis in AIDS patients was higher than that in patients with liver disease(χ2=9.843,P=0.043).Urine,ascites,bile and blood were the most common specimen types,and there was a statistical difference in the distribution of Candida species among different specimen types(P<0.05).Candida albicans and Candida tropicalis were sensitive to amphotericin B and 5-fluorocytosine,and the resistance rate of Candida tropicalis to triazole drugs(fluconazole,itraconazole and voriconazole)was higher than thsoe of Candida albicans,with statistical significance(P<0.05).In recent five years,the change of drug resistance rate of Candida albicans was relatively smooth.The drug resistance rate of Candida tropicalis to triazole drugs decreased from 2019 to 2020,and increased from 2020.The drug resistance rate of AIDS patients was higher than that of patients with liver diseases,and the differences were statistically significant(P<0.05).The detection of Candida auris began in 2023.Conclusions The proportion of Candida albicans in Candida bloodstream infection in infectious disease hospitals is relatively low.The drug resistance rate of Candida to triazole drugs is higher,among which the drug resistance rate of AIDS patients is higher than that of patients with liver disease.Clinical use of antibiotics should be reasonable according to different patients type and infection site.
