Literature analysis of electrolyte supplement in parenteral nutrition prescriptions in China from 2014 to 2023
Objective To understand the status of electrolyte supplement in parenteral nutrition(PN)prescription in China.Methods"Parenteral nutrition""intravenous nutrition""prescription analysis""rational drug use""analysis of medical order"were taken as keywords,reletvant literatures about the rationality analysis of PN prescriptions published from CNKI and Wanfang databases from 2014 to 2023 were selected,and the utilization of electrolyte included in literatures were analyzed.Results A total of 99 literatures(85 for adults and 14 for newborns)were included,involving a total of 839 026 prescriptions(810 870 for adults and 28 219 for newborns).Among the 99 literatures,86 literatures(72 for adults and 14 for newborns)confirmed that electrolyte was supplemented in PN prescriptions,involving 750 874 prescriptions(89.49%).Of the 72 adult PN literatures,16 literatures defined the variety supplement of cationic preparations,of which 10(62.5%)literatures supplemented four cations at the same time;while among the 810 807 adult PN prescriptions,722 655(89.13%)were supplemented with electrolytes,of which 6 261(0.87%)prescriptions were supplemented with electrolytes with excessive stability,and electrolyte replenishment was not regularly monitored in 70 literatures(97.22%).Of the 14 neonatal literatures,only 7.1%(1/14)confirmed supplement of four cations at the same time,while among the 28 219 neonatal PN prescriptions,28 219(100%)were supplemented with electrolytes,of which 516 prescriptions(1.83%)had excessive stability.Conclusions The proportion of standardized use of electrolytes in PN prescription in China is low,which is manifested by the fact that there is no electrolyte added in PN prescription,or the electrolyte is over-supplemented,and the selection of electrolyte is not standardized.It is suggested that the electrolyte supplement in PN prescription should be standardized and the electrolyte situation should be monitored regularly.
