Study on risk prediction model of thyroid dysfunction in patients with severe traumatic brain injury in one year after surgery
Objective To explore the influencing factors of thyroid dysfunction(TD)in patients with severe traumatic brain injury(sTBI)one year after surgery,and to construct a risk prediction model of TD.Methods A total of 137 patients who underwent sTBI in the Department of Neurosurgery of the First Affiliated Hospital of Air Force Military Medical University from June 2018 to June 2021 were selected,and the patients were followed up for one year after surgery.The general demographic information,disease-related information and laboratory examination data of the patients were collected.Multivariate logistic regression equation was used to analyze the influencing factors of postoperative TD in sTBI patients.The risk nomogram model of postoperative TD in sTBI patients was established,and ROC curve was used to evaluate the distinguishing ability of the model,and Bootstrap 1 000 repeated sampling method was used to verify the model internally.Results Among the 137 patients,86 were males and 51 were females,aged from 24 to 68 years,with an average age of(44.4±5.2)years.There were 91 cases of TD(66.42%)in sTBI patients who were followed up for one year after operation.Multivariate analysis results showed that sTBI patients with fasting blood-glucose(FBG)≥7.8mmol/L,number of operations≥three times,longer stay in ICU and higher levels of SCr were more likely to occur TD within one year after surgery.The AUC of the ROC curve of the risk prediction model of TD in sTBI patients one year after operation was 0.925(95%CI:0.839-0.962,P<0.05),and the calibration curve showed that the predicted TD occurrence probability of the model was consistent with the actual observation probability.Conclusions Based on FBG,number of operation,ICU stay time and levels of SCr,the risk prediction model of TD in sTBI patients in one year after surgery has good performance and has application value in screening high-risk groups of TD in sTBI patients in one year after surgery.
